- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E040127, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is set aside for Native American Indian Owned Businesses under the authority of the Buy Indian Act (25 U.S.C. 47). All responsible Native American Indian Owned business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 0001: Red Road Project Director: Provide all travel, supplies, materials, and personnel to provide a Red Road Project Director to the BIA, Flandreau Indian School, Flandreau, SD in accordance with the Scope of Work (SOW) beginning September 7, 2004 through May 26, 2005. The QUANTITY is One (1), The UNIT is Lump Sum. 0002: Red Road Project Director: Provide all travel, supplies, materials, and personnel to provide a Red Road Project Director to the BIA, Flandreau Indian School, Flandreau, SD in accordance with the Scope of Work (SOW) beginning on or about September 1, 2005 and ending on or about May 26, 2006. The QUANTITY is One (1), The UNIT is Lump Sum. The Government intends to award a firm fixed price contract. The period of performance is from September 7, 2004 through May 26, 2005 with one option year period. The SCOPE OF WORK (SOW) is as follows: Scope of Work: Students at FIS come from varied and often socially, physically or emotionally traumatic and abusive backgrounds. These students are in need of intervention and counseling in order to develop into healthy, productive and contributing members of their communities. This position will be responsible for providing one avenue in a multi-faceted approach to intervention, counseling and self development. Students are in need of evaluation, counseling and assistance in individual growth and development of healthy self-concepts. Objectives: Evaluate and screen students at Flandreau Indian School with other counseling programs and staff and provide counseling and cultural values based guidance as cases indicate. Contractor tasks: the contractor will have a counseling background; have experience working with at-risk Native American students; confer with counseling, academic and homeliving staff; meet with students individually or in group settings using a variety of counseling methodologies and concentrating on values-based cultural approaches; document student progress and participation as necessary for grant evaluation requirements; work with grant director for ascertaining documentation and evaluation procedures; meet with faculty and staff to assist in discussions and decisions relating to student placement and evaluation; perform the required duties both within and outside of the regular school day, evenings and weekends; meet with the grant director on a regular basis (timing to be determined by therapist and director) as program develops. Contract end items: Starting date is September 7, 2004. End date is May 26, 2005, with one option year period. Director will provide all reports necessary for grant reporting and evaluation. Information collected will not violate any confidentiality or counselor ethics requirement, but will only meet grant requirements. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award. This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given if full text.
