Description

Brooke Army Medical Center, Fort Sam Houston, Texas. The Government intends to negotiate on a sole source basis with Perfusion Resources Networks I LP, 2320 Michigan Court, Arlington, Texas for Custom Open Heart Perfusion Packs that are utilized to keep blood circulating in the patients body while undergoing heart bypass surgery. The Packs include the following equipment: (a) 1 each, Urine Meter Foley Trap w/temp Sensor RSP (SIMS), 16 Fr [#UM400-16]; (b) 2 each, DLP Cardia Suckers [#10061]; (c) 1 each, Trace Cart, [DeRoyal Industries #53-1094]; (d) 1 each, Pressure Monitoring Line 84 inch male and female ends [Medex MX567]; (e) 1 each, Tubing pack to specifications to include Affinity Membrane Oxygenator and Medtronic BP-80 Centrifugal Pump. Quantities to be purchased are estimated from 20 Packs to 170 Packs. Packs will be ordered on an as needed basis. The period of performance is from date of award through September 30, 2005. This procurement is being conducted as a commercial item acquis ition in accordance with FAR Part 12. All responsible sources may submit and offer, which will be considered. Solicitation W81K00-04-T-0565 will be available at https://acquisition.army.mil.asfi/ on or about September 7, 2004. Paper copies of this solic itation will not be issued and telephone requests or fax requests for this solicitation will not be accepted. Award to be made on or about September 13, 2004.
