- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0060, is issued as a request for quotation (RFQ). This acquisition is a total small business set-aside and the order will be a firm fixed price order. NAICS: 325413. All responsible businesses may submit a quote that shall be considered. All inquiries and/or quotations should be directed to the purchasing agent, Kohala Webster, Naval Medical Center San Diego, 34800 Bob Wilson Drive, San Diego, CA, 92134, or e-mail: kowebster@nmcsd.med.navy.mil or fax 619-532-5596 no later than 17 Sep, 2004, 2:30 p.m. PDT. No phone inquiries will be accepted. In accordance with FAR 52.212-2, Evaluation - Commercial Items (1999), award will be made based on technical capability, past performance, and price. These evaluation factors have equal levels of importance. For technical capability, to be considered for award, offers of ?equal? products, including ?equal? products of the brand name manufacturer, must meet the salient physical, functional, or performance characteristic specified in this solicitation; clearly identify the item by Brand name, if any, and make or model number, and include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer. For past performance evaluation, provide at least three (3) references of sources for whom you have provided the specified materials and/or service(s). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2004-24. This is a one year Standing Order for several deliveries throughout the contract period, the total quantity and number of deliveries being shown for each CLIN.
