Description
Request for Quotation Upgrade Video/Audio Systems -----THIS AMENDMENT IS ISSUED TO EXTEND THE QUOTATION DUE DATE TO 1:30 P.M., MOUNTAIN TIME 20 SEPTEMBER 2004.----- This is a combined synopsis/solicitation for commercial items prepared in accordance with Sub part 12.6, as supplemented with additional information included in this notice. The solicitation number is H94002-04-Q-0014 and is issued as a Request for Quotation (RFQ). FOB Destination shall be Aurora, CO, 80011. The Government will choose that quotation which is most advantageous. This contract action will result in a Firm Fixed Price contract, contingent upon availability of funds. This requirement is set aside 100% for small business. Interested sources must submit in writing, technical capabilities and other information that demonstrates their ability to meet this requirement. Inquiries and request for quotation packages must be in writing and submitted to dan.poturalski@tma.osd.mil. Vendors should be registered in Central Contractor Registration (www.ccr.gov) before award can be made. All quotations must be received no later than 1:30 P.M., Mountain Time on 10 September 2004. Send all quotations to Mr. Daniel H. Poturalski, Office of the Assistant Secretary of Defense Health Affairs, TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, Colorado 80011-0943, or by facsimile to (303) 676-3987. Facsimile quotes should be followed by signed, hardcopy forwarded to the address stated above. The quotation submitted by the contractor against this requirement shall clearly state the following: Request for Quotation Number; Time specified for receipt of offers; Name, Address; Telephone number of Offeror; Terms of warranty; price, and discount terms and acknowledgement of all quotation amendments (if applicable). TRICARE requires the following: 1. INTRODUCTION 1.1 TRICARE Management Activity (TMA) currently has audio/video equipment in four of its rooms in the facility located at 16401 East Centretech Parkway, Aurora, CO 80011-0943. These rooms are located as follows: Program Integrity Conference Room #137 - 1st Floor Program Requirements Conference Room #217 - 2nd Floor Executive Conference Room #232 - 2nd Floor Office of the General Counsel Conference Room #241 - 2nd Floor It is the intent of TMA to procure upgraded audio/video systems and equipment and to have this equipment installed by the contractor in these rooms. 1.2 Interested Contractors are invited to a site visit to examine the rooms, view the existing equipment, and get a better perspective of the performance requirements at TMA. Contractors will be requested to construct a design fulfilling the intent of these requirements and to submit a proposal. The date and time for this site visit is September 8, 2004 at 2 pm at the TMA facility. Interested Contractors will contact Mr. Daniel H. Poturalski via e-mail. Due to time constraints, questions resulting from the site visit will not be considered after 12:00 pm MST, September 9, 2004. 2. OBJECTIVE 2.1 Room 232 Up-grade audio/video systems and equipment in the Executive Conference Room 232 to provide high quality audio and video presentation. To integrate central control and operational management of the various systems (VTC, computer, DVD/VCR, internet display on the plasma display or projected on to the room screen, as well as display of other computer programs and applications, high quality audio via ceiling mounted speakers). Room 232 is 24 ft x 28 ft. Equipment will be arranged in the Northeast corner of the room, across the corner facing the conference table. 2.2 Room 137 Up-grade audio/video systems and equipment in the Program Integrity Conference Room 137 to provide high quality audio and video presentation. Room 137 is 12 ft x 23 ft. Equipment will be arranged in the Southwest corner of the room, across the corner facing the conference table. 2.3 Room 217 Up-grade audio/video systems and equipment in the Program Requirements Conference Room 217 to provide high quality audio and video presentation. Room 217 is 13 ft x 21 ft. Equipment will be arranged in the Northeast corner of the room, across the corner facing the conference table. 2.4 Room 241 Up-grade audio/video systems and equipment in the Office of the General Counsel Conference Room 217 to provide high quality audio and video presentation. Room 241 is 15 ft x 16 ft. Equipment will be arranged in the Northeast corner of the room, across the corner facing the conference table. 3. Performance Requirements 3.1 The plasma display units shall not be wall mounted. The units shall accommodate a set-top video conferencing codec. (Government furnished Polycom View Station FX H.323) The bottom of the unit should be no further than 40 to 50 inches from the floor. 3.2 The Contractor shall be responsible for all installation to include any design engineering, programming, documentation, installation labor, cabling and power to accommodate proper working and integration of all systems (Contractor will be responsible for bundling and concealing so that cables are as inconspicuous as possible), training and warranty. 3.3 Equipment 3.3.1 Video: Plasma Display, multicolor, high resolution. 42? to 60? screen, 2 ea. (Rms.232 and 137) 36? to 42? screen, 2 ea. (Rms. 241 and 217) (with side mounted stereo speakers) 3.3.2 Audio/Video Switcher, 4 ea. (Rms. 232, 137, 241 and 217) 3.3.3 Computer /Video Interface: (Rms. 232, 137, 241 and 217) High Resolution, compact, 4 ea. 3.3.4 Wireless keyboard and Ultra mouse, 4 ea. (Rms. 232, 137, 241 and 217) 3.3.5 Storage Units, 4 ea. (Rms. 232, 137, 241 and 217) 3.3.6 Audio: Mixer/Amplifier, 2 ea. (Rms. 232 and 137) Distribution Amplifier, 1 ea. (Rm. 232) Ceiling Speakers, 10 ea. (Rms. 232-6ea, 137-4 ea.) 3.3.7 Control System: Control System, compact with fixed resource control and external power supply, 1 ea. (Rm. 232) Touch Panel with transceiver/wireless gateway, two way wireless, color, digital volume control, audio matrix mixer, equalizer and infra-red emitter probe, 1 ea. (Rm. 232) 52.236- 3 Site Investigation and Conditions Affecting the Work (Apr 1984) (a) The Contractor acknowledges that it has taken steps reasonably necessary to ascertain the nature and location of the work, and that it has investigated and satisfied itself as to the general and local conditions which can affect the work or its cost, including but not limited to (1) conditions bearing upon transportation, disposal, handling, and storage of materials; (2) the availability of labor, water, electric power, and roads; (3) uncertainties of weather, river stages, tides, or similar physical conditions at the site; (4) the conformation and conditions of the ground; and (5) the character of equipment and facilities needed preliminary to and during work performance. The Contractor also acknowledges that it has satisfied itself as to the character, quality, and quantity of surface and subsurface materials or obstacles to be encountered insofar as this information is reasonably ascertainable from an inspection of the site, including all exploratory work done by the Government, as well as from the drawings and specifications made a part of this contract. Any failure of the Contractor to take the actions described and acknowledged in this paragraph will not relieve the Contractor from the responsibility for estimating properly the difficulty and cost of successfully performing the work, or for proceeding to successfully perform the work without additional expense to the Government. (b) The Government assumes no responsibility for any conclusions or interpretations made by the Contractor based on the information made available by the Government. Nor does the Government assume responsibility for any understanding reached or representation made concerning conditions which can affect the work by any of its officers or agents before the execution of this contract, unless that understanding or representation is expressly stated in this contract.
