Description

The purpose of this amendment is to extend the response date. If any quotes were e-mailed on 8 September 2004, due to system outage quotes may not have been received. Please resubmit by fax at 808-473-2522 no later than 10 September 2004, 11:00 A.M. Hawaii Standard Time. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as request for proposal No. HC1019-04-Q-0006. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. This acquisition is solicited on a small business set aside basis.
