- Description
- The Bureau of Prisons is seeking a Request for Quote from sources having the ability to provide OUT-PATIENT assessment and counseling services in the Rockford, IL and surrounding areas, for the drug abuse treatment, and mental health treatment, of male and female Federal offenders (inmates) held under the authority of various United States Statutes. Performance period under this contract will be for a one year base period to begin on October 1, 2005 with four (4) one year option periods, with service ending (if all option years are renewed) on September 30, 2010. The services required for this contract include: Base Period: October 1, 2005 to September 30, 2006. Estimated Number of Inmates 5. (2) Intake Assessments (only two of the five inmates are estimated to need assessments); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 5 inmates = 240 total units; Group Counseling Sessions (30 minute sessions at 6 units per month x 12 months) 72 Group Units x 5 inmates = 360 total units.(1) Mental Health Intake Assessment and Report (only one of the five inmates are estimated to need assessments); (1) Medication Monitoring (only one of the five inmates are estimated to need medication monitoring). Option Year One: October 1, 2006 to September 30, 2007. Estimated Number of Inmates 5. (2) Intake Assessments (only two of the five inmates are estimated to need assessments); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 5 inmates = 240 total units; Group Counseling Sessions (30 minute sessions at 6 units per month x 12 months) 72 Group Units x 5 inmates = 360 total units.(1) Mental Health Intake Assessment and Report (only one of the five inmates are estimated to need assessments); (1) Medication Monitoring (only one of the five inmates are estimated to need medication monitoring). (2) Intake Assessments (only two of the six inmates are estimated to need assessments); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 6 inmates = 288 total units; Group Counseling Sessions (30 minute sessions at 6 units per month x 12 months) 72 Group Units x 6 inmates = 432 total units.(1) Mental Health Intake Assessment and Report (only one of the six inmates are estimated to need assessments); (1) Medication Monitoring (only one of the six inmates are estimated to need medication monitoring). Option Year Three: October 1, 2008 to September 30, 2009. Estimated Number of Inmates 7. (3) Intake Assessments (only three of the seven inmates are estimated to need assessments); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 7 inmates = 336 total units; Group Counseling Sessions (30 minute sessions at 6 units per month x 12 months) 72 Group Units x 7 inmates = 504 total units. (1) Mental Health Intake Assessment and Report (only one of the seven inmates are estimated to need assessments); (1) Medication Monitoring (only one of the seven inmates are estimated to need medication monitoring). Option Year Four: October 1, 2009 to September 30, 2010. Estimated Number of inmates 8. (4) Intake Assessments (only four of the eight inmates are estimated to need assessments); Individual Counseling Sessions (30 minute sessions per inmate) 48 Individual units x 8 inmates = 384 total units; Group Counseling Sessions (30 minute sessions at 6 units per month x 12 months) 72 Group Units x 8 inmates = 576 total units. (1) Mental Health Intake Assessment and Report (only one of the eight inmates are estimated to need assessments); (1) Medication Monitoring (only one of the eight inmates are estimated to need medication monitoring). Note: The Mental Health Services listed may be provided by the contractor or sub-contracted to another contractor. The services described shall be in accordance with the Federal Bureau of Prisons Community Transitional Drug Abuse treatment (TDAT) Statement of Work, dated January 2004. The estimated closing date is o/a November 17, 2004. Faith-Based and Community-Based Organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible. All responsible sources may submit an offeror which will be considered by this Agency. The RFQ 200-049-4-NC will be posted o/a September 14, 2004.
