Description
Subject to the availability of funds, the Department of Veterans Affairs Medical Center, 2495 Shreveport Highway, Alexandria LA, plans to negotiate a follow-on, sole source agreement with Nova Biomedical, 200 Prospect St, Waltham MA 02254-9141 for preventive and full maintenance on 2 PHOX computerized blood gas analyzers, as follows: (a) Unlimited Emergency Service Calls relating to electronic, mechanical, or instrument malfunctions; (b) Two (2) preventive maintenance calls with in-service training/product improvement updates, as appropriate (c) All mechanical and electronic parts excluding consumables; (d) Loaner instrument if necessary; & (e) Software updates. All services shall be in accordance with the original equipment manufacturers (OEM) recommendations. All replacement parts, kits, hardware, software, revisions, updates, and modifications shall meet or exceed OEM specifications on the following Physiological Gas Analyzers NOVA PHOX, SERIAL # U01503160 & NOVA PHOX, SERIAL # U01303120. Repair technician shall be trained and certified on specific equipment listed above. Technician shall provide all tools, software, expertise, certification of training, and any other special equipment, parts or supplies, and expertise to repair, calibrate, certify and maintain Nova PHOX Blood Analyzer equip equipment. Software updates shall be provided and installed at no charges as they become available. There is no other known source to provide the services and software to ensure this critical medical equipment is properly maintained, inspections performed and certificates of same provided to meet JCAHO and VA requirements. This procurement is subject to the availability of FY 2005 funds for base year and annual thereafter. NAICS Code is 811219, Other Electronic & Precision Equipment repair & Maintenance. Any contractor claiming to possess the required qualifications shall contract the contracting officer immediately at 318-473-0010, ext 2240. See numbered note 22 -- changed to require inquiries be submitted to Contracting Officer no later than September 20, 2004.
