(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quote is being requested and a written solicitation will not be issued. (ii) Solicitation Number DHQCNT04MP60026. This solicitation is a Request For Quote (RFQ). (iii) The RFQ document and incorporated provisions and clauses are those in effect through FAC 2001-24 and DFARS Change Notice (DCN) 20040625. (iv) This is a SMALL BUSINESS SET-ASIDE. NAICS Code 541519 Other Computer Related Services. Size standard $21M. (v) Provide a price for contract line item numbers (CLINs) listed in the attached Excel Spreadsheet, Addendum 1, Schedule of Supplies/Services. (vi) Description of Requirement: Acquisition of Mainframe Remote Management Solution to include hardware, software and services. See Addendum 2, Statement of Work. (vii) Period of performance shall be one base year plus four one-year options. Reference Addendum 2 for delivery date and locations. (viii) FAR 52.212-1 applies to this acquisition. FAR 52.212-1(b)(4): Offerors shall submit existing product literature and software license in lieu of unique written technical quotes. FAR 52.212-1(b)(10): Reference Addendum 2, Paragraphs 9.4. and 15.1.2. (ix) See Addendum 2, Paragraph 15 for Vendor Evaluation Criteria and Addendum 3, Phase I - Evaluation Plan and Addendum 4, Phase II - Evaluation Factors. (x) Quote shall include a completed copy of the provisions at FAR 52.212-3 and DFARS 252.212-7000 and 252.247-7022. (xi) FAR 52.212-4 applies to this acquisition. (xii) FAR 52.212-5 applies to this acquisition ? FAR 52.212-5(b): The following additional FAR clauses apply to this acquisition: 52.219-6; 52.222-26; 52.222-35; 52.222-36; 52.222-37. (xiii) Additional requirements: (1) The following FAR clauses apply to this acquisition: 52.217-8; 52.217-9; 52.232-33; 52.252-2 (access to clauses can be made via www.farsite.hill.af.mil); (2) DFARS 252.212-7001 applies to this acquisition ?
