- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number FA4600-04-T-0202 is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-18. All applicable provision and clauses are included in the actual solicitation. This electronic announcement constitutes the only solicitation being issued. This procurement is set aside 100% for small business. The North American Industry Classification System (NAICS) code is 238330, and the small business size standard is $12M. The Government intends to issue a firm fixed price purchase contract for the following: 1. 9MM (3/8?) Rolled Rubber Flooring ? Solid Black (QTY 9588). 2. Parabound Adhesive M-4700, 4-Gal Pails (QTY 27). 3. Shipping/Installation Statement of work is as follows: Service: Remove existing free weight room flooring and carpet Frady Fitness Center and install new floor at Offutt Field House. Job Site: Frady Fitness Center bldg 305, Offutt Field House bldg 321 Offutt AFB NE 68113. Pre renovation procedures: Fitness Center will move all equipment at Frady fitness center so contractor can perform work. Offutt Field House location is new area -relocation of batting cage will be accomplished so contractor can perform work. Scope of work: Frady Fitness Center - contractor will have to remove all existing weight room flooring which consists of both carpet and rubber based flooring. Contractor will need to reinstall new flooring to include any preparations, floor repairs, transition materials, cove base moldings and adhering new product in place at both the OFH and Frady Fitness Center. Areas to be covered at the OFH and Frady fitness center are 86? x 60? and 81? x 51?. Floor covering has to be a minimum of 9mm (3/8?) Rubberized shock absorbent material Material cannot retain or absorb sweat or discolor due to sweat and moisture Material must be easy to maintain and disinfect Width 48? Color solid black Floor covering should be in the form of rolls vs. individual squares for shifting and separation purposes. Contractor will be responsible for disposing of all materials removed from Frady as well as ensuring all materials; debris and dust are removed from each facility upon completion of work. The following are also applicable to this acquisition, DFAR 252.204-7003 Control of Government Personnel Work Product, DFAR 252.204-7004 Alt A (Nov 2003) Required Central Contracting Registration Alternate A, DFAR 252.232-7003 Electronic Submission of Payment Requests, and DFAR 252.243-7001 Pricing of Contract Modifications. A MANDATORY site visit is scheduled for Tuesday, 21 September 2004 at 10:00 a.m. CST at the Field House on Offutt AFB, NE. All contractors who plan on attending MUST send a confirmation e-mail to me at jennifer.hoffman@offutt.af.mil NLT 12:00 p.m. CST on Monday, 20 September 2004. Bids will not be accepted from Contractors who do not attend the site visit. All quotations must be received by the 55 Contracting Squadron, 101 Washington Square, Offutt AFB 68113-2107 by Friday, 24 September 2004 at 12:00 p.m. CST.
