- 30th Space Wing, Vandenberg AFB, CA is seeking sources who can provide oral surgeon services on-site at Vandenberg AFB, CA. The Statement of Work is as follows: A. The contractor shall provide oral surgery services at the 30th Medical Group, Vandenberg AFB, California. The contractor shall provide on-site service to personnel two (2) mornings per month. The contractor: 1. Must comply with the standards of the Joint Commission on the Accreditation of Healthcare Organizations. 2. Shall provide, upon request, quality data and information related to the services provided that support both the Performance Improvement Program and the Risk Management process. 3. Must have a dental degree from an ADA recognized institution. 4. Must have an active and current dental license in at least one jurisdiction. 5. Must maintain current BLS credentials. B. Services will include the surgical removal of impacted teeth, biopsies, alvelolectomies, consultations and follow-up of systemic conditions that have oral involvement. An average morning might include a few consultations, 2 or 3 sets of impacted third molars, a biopsy, or similar services. C. Under this contract's SOW/PWS, the contractor will be considered a 'Business Associate' under the terms of the Health Insurance Portability & Accountability Act of 1996 (HIPAA), Public Law 104-191, and its implementing regulations, including DoD 6025.18-R. In the performance of this contract, the contractor shall, when performing as a 'Business Associate', comply with all rules and requirements applicable to a 'Business Associate' under HIPAA and its implementing regulations, including DoD 6025.18-R. Violation of HIPAA may involve the imposition of fines and/or imprisonment. The contractor shall include this notification and requirement in every subcontract for 'Business Associate' service. The proposed contract action is for supplies or services for which the government intends to solicit and negotiate with only one source under the authority of FAR 13.106. Interested persons may identify their interest and capability to respond to the requirement or submit offers. This notice of intent is not a request for competitive offers. However, the government will consider all offers received within seven (7) days after the publication date of this synopsis. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
