- Amendment: The purpose of this amendment is to extend the due date for responses from 4pm 20 Sept 2004 until 4pm 22 Sept 2004 and to state this contract will be for 1 base year with 4 options years. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplement with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Reference number for request for proposal (RFP) is F73MSO41541300. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. This solicitation will be 100% set aside for small business. The NAICS code is 561421 and size standard is $6,000,000.00. Non-personal Services: Provide telephone answering service (alphanumeric dispatch, voice mail, wakeup service, patching, and fax delivery) for the Maxwell Ambulatory Healthcare Clinic. A live attendant is required at Contractor"s location from 1600-0700 Moday through Friday; 24 hours a day, Saturday, Sunday, and all federal holidays. The requirement will be conducted in accordance with the attached Performance Work Statement. Clauses and provisions can be accessed via the Internet at http://farsite.hill.af.mil.
