Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request For Quotation no. W9128A-04-T-0039 is issued as a firm-fixed price commercial services requirement for Relocation and Installation of Audio Visual System at the Pacific Ocean Division Emergency Operations Center, Building 525, Fort Shafter, Hawaii. Work includes all necessary management, labor, supervision, materials, equipment, transportation, and any other incidental services required to remove, relocate, and install an Audio Visual (A/V) system from existing rooms on the 3rd floor of Building 525 to other rooms on the 3rd floor of Building 525. Major work tasks include preparation and submission of a Safety Plan; field investigation; verification of A/V system operation status; preparation and submission of a Work Plan; coordination with site cons truction contractor who is performing renovation work; removal, installation, and connection of A/V equipment and server; operational testing of the A/V system; preparation and submission of as-built plans, maintenance guide, and source code; and a one (1) year warranty to include labor and materials for the work. All installation, connection and testing of all data cabling shall be accomplished by certified CAT 6 technicians. The Contractor is liable for all damages to materials and property incurred durin g the course of work. Onsite work shall commence no earlier than November 1, 2004 and shall be completed by January 31, 2005. The Contractor shall furnish three (3) copies of the deliverables in electronic format on compact disks. Deliverables shall be sub mitted within thirty (30) calendar days of Government acceptance. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. Provisions at (1) 52.212-1, Instructions to Offerors  Comme rcial; (2) 52.212-2, Evaluation -- Commercial Items where evaluation is based on Price and Technical and Past Performance, when combined, are compared to Price; (3) 52.212-4, Contract Terms and Conditions -- Commercial Items; and (4) 52.212-5, Contract Ter ms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items apply to this acquisition. A Justification and Approval citing 10 U.S.C. 2304(c)(1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements has been approved. Multimedia Solutions (MS) is the proposed sole source contractor who is the only sour ce that can accomplish this work. MS provided and installed the system as a subcontractor under Contract no. DACW83-00-C-0061. MS owns the proprietary source code for the existing A/V system. The source code is required to perform security checks, troubles hoot, reconfigure, and maintain the system; these functions cannot be accessed or performed without the source code. Utilizing any other source may jeopardize the reliability of the AV system, increase installation/acceptance time, and cause substantial du plication of cost to the Government. A completed copy of the provision at 52.212-3, Offeror Representations and Certifications  Commercial Items must be included with the proposal.
