RFP-R-40-04DC NAICS Code: 811212 100% Small Business Set Aside This requirement includes a SF 1449 issued by USDA/FSA/MSD/Acquisition Management Branch: This requirement is based upon the following CLINS and Statement of Work: 0001: BASE YEAR: 1 OCT 2004 – 30 SEP 2005 Copier Maintenance & Supplies 0002: OPTION YEAR I: 1 OCT 2005 – 30 SEP 2006 Copier Maintenance & Supplies 0003: OPTION YEAR II: 1 OCT 2006 – 30 SEP 2007 Copier Maintenance & Supplies 0004: OPTION YEAR III: 1 OCT 2007 – 30 SEP 2008 Copier Maintenance & Supplies 0005: OPTION YEAR IV: 1 OCT 2008 – 30 SEP 2009 Copier Maintenance & Supplies Statement of Work: 1. Scope The Contractor shall be responsible for providing all labor, materials, supplies and supervision (except as noted in 2. II Technical Support below) necessary to perform on-site copier/digital equipment, maintenance and supplies for the current installed base of equipment in all categories as described in Attachment A, as well as, for new equipment added to any of these programs during the course of the contract. All service representatives shall be factory trained and carry 100% of all necessary tools including equipment to service all copiers. Presently the copier inventory is in good to excellent condition and has been under a continuous maintenance program. The copiers are located in five buildings in the Washington metropolitan area including Northern Virginia (See Attachment A). FSA reserves the right to change or move machines during the length of this contract, however, any change in the total number of machines will be made by a contract modification. 2. Technical Support The Contractor’s Service support personnel shall maintain all equipment at the current locations to factory specification levels in a professional and competent manner. The contractor shall provide on-site service and support to all locations. Support tasks shall include the following: I. Contractor shall provide a dedicated 800 telephone number or e-mail address for receiving service notification calls. Onsite service shall be rendered during normal business hours 8am – 5pm, Monday through Friday. II. The contractor shall provide all parts, supplies and consumables for copiers/digital equipment. The USDA FSA will furnish all paper, binding materials, tabs, and other non–copier/digital print engine consumables. Only original equipment manufacturer’s supplies shall be used. All service calls shall be responded to with on-site service within 4 hours of the service request. Any service call requiring additional time to place the machine in service shall be communicated to the COTR by the on-site contractor or technician within one hour of service request. Contractor shall not allow the supplies for any machine to be depleted at any time. 3. Reporting: The contractor shall produce a monthly report by location for the USDA-FSA COTR with the following information a. Meter reads by location and unit b. Service calls by location and unit c. Response time by location and unit d. Average up-time by location and unit e. Monthly volume by location and unit Note: The contractor shall automatically deduct 2% from any monthly invoice where the average response time exceeds 4 hours for the entire inventory of equipment. This report shall be delivered to Ms. Jessie Bryan/USDA/FSA/MSD/1400 Independence Ave, S.W.-STOP 0563/Washington, DC 20250-0563 and to Ms. Sandra Garland, Contracting Officer, USDA/FSA/MSD/AMB/SPS 1400 Independence Avenue, S.W., Stop 0567/Washington, DC 20250-0567. 4. CONTRACT DURATION AND PRICING The contractor shall provide pricing information for the installed base of USDA-Owned equipment in Attachment A. The contractor shall submit a quality control plan. Contract will be firm fixed price for base year and four (4) USDA FSA option years. Please provide firm fixed pricing for each of FY’s 05, 06, 07, 08 and 09. Note: Monthly image allowances are in aggregate for the entire installed base and overages may not be charged unless the maximum installed base is exceeded. 5. ATTACHMENTS A. BASIS FOR AWARD The proposal shall demonstrate successful past performance experience in the areas described in the statement of work. The proposal shall describe performances of same or similar efforts in scope and complexity completed within the past five (5) years using the Performance Data Template provided as Attachment D. In the event no Past Performance information is available, a neutral rating shall be given.
