- : This is a combined synopsis/solicitation. The VA Network Business Center Logistics Office, Long Beach, California intends to negotiate a sole source contract with Northwest Surgical Products, 1221 Pureta De Sol, Suite 100, San Clemente, CA 92673. This BOS announcement constitutes the only solicitation issued. This procurement is being conducted in accordance FAR 6.302 - only one responsible source and no other supplies or services will satisfy agency requirements NAICS 421450. Requirements include the following: OSI Vascular imagining Table. NOTE: ALL THE FOLLOWING EQUIPMENT IS RELATED AND INTERLOCKS. (1) (Stock # 6860) Vascular Imaging Table with 4-way Float the equipment shall include the following futures (a) 400Lbs. (182Kg) patient weight capacity, (b) Carbon fiber table top (c) battery back-up system (c) Foot Pedal (d) Joystick, power hand control with 3 position memory feature, includes return to level and 2 other programmable positions (e) Patient safety straps (f) Standard Or Accessory rails (g) Extension OR accessory rails (h) 2 in mattress standard (i) 2in mattress standard (j) 100V electrical system or 220V (specify international) . (2) The Vascular Imaging Table equipment shall have the following specifications: (a) table top length 102 in (259 cm) (b) Table top width 24 in (61cm) (c) Free and Clear imaging Area 57 in, (145cm)(d) Table Top Height Range 34in. 40in. (e) Trandelenburg /Reverse Trendelenburg 7.5 ?15 degree (f) lateral title 15 degree (g) Longinal top travel 38 in. (h) Table Top Movement Float. Proposals shall be submitted on an "all or none basis." Partial proposals shall be rejected. If submitting on a brand name equal basis, sufficient technical data must be included for the government's evaluation. This purchase shall also include FOB destination delivery provisions, required installation and a one-year warranty, all at no charge. Delivery and installation shall be at VA San Diego Healthcare System, 3350 La Jolla Drive, Building 1, O. R , San Diego, California 92161. Offeror shall submit a complete copy of provision FAR 52.212-3, Offeror Representation and Certification- Commercial Items (Jan 2004). In paragraph (b) of 52.212-5 the following apply: 52.222-19, 52.222-21, 52.222-36, 52.222-37, 52.222-19, 52.225-1, 52.225-3, 52.225-15, and 52.232-34. 52.222-70 COMPLIANCE WITH VETERANS EMPLOYMENT REPORTING REQUIREMENTS (JAN 1999)(DEVIATION)(USDA). (In addition, incorporated herein by reference are the following VA Acquisition Regulations (VAAR) clauses and provisions: 852.211-70 Service Data Manuals (Nov 1984), 852.211-71 Guarantee (Nov 1984); 852.270-1 Representative of the Contracting Officer (April 1984) and 852.270-1 Commercial Advertising (April 1984) . This notice of intent is not a request for competitive quotes however; any firm any firm that believes it can meet the requirements may give written notification to the contracting officer within six days from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information can be sent to the Contracting Officer. A determination by the Government not to compete this proposed contract based on responses to this notice is solely with the discretion of the Government.
