Description

The Federal Bureau of Prisons, Field Acquisition Office in Grand Prairie, Texas intends to make a single award for the provision of a Dental Hygienist for the Federal Correctional Institution, Schuylkill, Pennsylvania, located at Interstate 81 and 901W., Minersville, PA 17954. Services to include: dental health education, dental prophylaxis and topical fluoride application,dental screening examination, subgingival scaling and preparation of patients for periodontal surgery, root planning, soft tissue curettage, dental charting, periodontal charting, prescribed treatment of localized alveolar osteitis and pericoronitis, preparing placing and removing periodontal packs, placing temporary fillings, polishing amalgam and composite restoration, application of pit and fissure sealants, placing and removing rubber dams, and removing sutures. Contractor must have passed written national Boards, clinical Regional Boards, be a registered Dental Hygienist, and be licenced to practice in the State of Pennsylvania. The contract shall consist of a base year with four (4) twelve (12) month option periods for renewal. The estimated quantities are 1,560 (1) one hour sessions for the base period and each option period excluding Federal Holidays (not to exceed 30 hours per week). The solicitation will be made available on or about October 4, 2004 and will be distributed solely through the General Administrations Federal Business Opportunities website at http://www.fedbizopps.gov/. Hard copies of the solicitation will not be available. The site provides downloading instructions. All future information regarding this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The anticipated date for receipt of quotes is November 4, 2004. Anticipated date of award is December 15, 2004, with performance anticipated to begin January 31, 2005. This solicitation is 100% set-aside for small business concerns. All responsible sources may submit a quote which will be considered. Faith-Based and Community-Based Organizations have the right to submit offers/bids equally with other organizations for contracts for which they are eligible.
