Description
Following are questions and replies submitted today are for information purposes. This will not change the response date. 1. If 1000 inches of loose medical record material is the minimum, how much do you actually have, and if significant, is there consideration of lengthening the contract to ensure completion of all backlog that currently exists at all three locations? 1. We are still looking at 1000 inches as a minimum. No consideration on lengthening the contract. 2. Will we be working with your internal staff to complete this project or will we be on our own? 2. Initial instructions will be provided to the contractor. After the introduction phase, the contractor will be on their own. 3. Are all 1000 inches of loose records in one location or are they spread between the three (Wilford Hall, Lackland AFB and Kelly AFB)? 3. Primarily focus on WHMC and other locations are secondary as time permits. 4. Will there be sufficient space for quantity (estimated at 12 people) of required personnel to perform the work in the time frame you have specified? 4. We only requested 3-FTE personnel and they will be sharing office space with our internal staff. 5. Can you provide examples or descriptions of exactly what we would be filing (i.e. clinical chart documentation, laboratory reports, x-rays, operative reports, surgery records, financial data)? 5. During the initial introduction, internal staff will provide any additional information on specific medical documents/forms. Note: AFI 41-210 (Patient Administration Functions), para 6.3.6 states the arrangement of content and record filing methodology in maintaining the Health Records. 6. Since review of medical record processes is desired, do you also wish for a review/analysis of ALL medical records at the three locations (Wilford Hall, Lackland AFB, and Kelly AFB) to ensure compliance with proper documentation order (contract requirement 2.4.2.5.) If you do not want a full review, do you want a sampling review/analysis to be completed? 6. We request a full review/analysis on our health records process.
