Description
This is a combined synopsis/solicitation for services prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 37.1, as supplemented with additional information included in this notice. The solicitation number for this Request f or Proposals is W9114F-04-R-0027. This solicitation is issued on an unrestricted basis under NAICS number 621112. DESCRIPTION : Personal Service  Pediatrician. Performance which shall occur at Heidelberg Medical Activity, Heidelberg, Germany. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52. 212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a conrtract without discussions with offerors. Therefore, the offerors initial offer should contain the offerors best terms f rom a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The governments objective is to obtain services from a contractor through a Personal Services Contract, who can perform the statement of work outline in the solicitation. Period of performance shall be for a twelve (12) month period from the date of award, with the option for continued performance. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and CertificationCommercial Items. An authorized representative of the offeror must sign these representations and certifications. The offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and ConditionsCommercial Items. Clause 52.212-5, Contr act Terms and Conditions Required to Implement Statutes or Executive OrdersCommercial Items, applies to this acquisition.
