- This notice is for informational purposes only. The Centers for Disease Control and Prevention intends to issue a sole source purchase order to Emory University, 80 Jesse Hill Jr. Drive, Atlanta, GA 30303 for fellow services. The physician will be in an accredited medical toxicology post-graduate medical education program. Although several such programs exist, the only one near enough to NCEH/ATSDR headquarters is that sponsored by Emory University. Fellows function as resident physicians and provide medical consultation and advice on public health problems in toxicology. In so doing, they enormously extend the impact of the FTE medical staff. Such consultations may involve development of medical aspects of protocols for research, investigational new drug applications, advice to public health workers in states and localities regarding toxicologic problems, and the development of educational materials on toxicologic problems for both professional audiences and the public. The quotation must reference Solicitation number 2005-Q-15576. Quotations will be due fifteen (5) calendar days from the publication date of this synopsis or October 4, 2004, by 8:30 a.m. local time, whichever is later. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to the Centers for Disease Control and Prevention, 2920 Brandywine Road, Atlanta, GA 30341, Attention: Linda T. Robinson. Email quotations will only be accepted if dated and signed by an authorized company representative to: ltr1@cdc.gov. This action is pursuant to 41 USC 253(1) and under the authority of FAR 6.302-1. The requirement will be procured as a commercial item in accordance with FAR Part 12.102(c) under the test program. Quotes will be due August 30, 2004, by 8:30 AM EST at the Centers for Disease Control and Prevention; Facilities and Construction Management Branch; Attn: Linda T. Robinson; 2920 Brandywine Road, Room 3000, Atlanta, GA 30341. Facsimile quotes will be accepted at 770-488-2847. RFQ# 2005-Q-15576. Attn: Linda T. Robinson, shall be clearly marked on the envelope and/or facsimile cover page. All responsible sources that can meet the above requirement may submit a quote, which will be considered by the Agency. The Central Contracting Registry is a central repository of all Contractors who wish to do business with the Federal Government. All vendors who wish to bid on the requirement MUST be registered and validated in the CCR prior to award of this Simplified Acquisition. Vendor who submit a quotation are showing that they are registered or plan to do so.
