Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation Part 12. A written solicitation will be issued. Solicitation number 673-08-05 applies and is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. The North American Industrial Classification Code is 492210 and the business size standard is 1,500 employees. Provide all supervision, labor, and material to provide delivery of lab specimens, medical records, and x-ray films from the Orlando VA Healthcare Center, Orlando, FL, to the Kissimmee Community-Based Outpatient Clinic, Kissimmee, FL, in accordance with the terms and provisions of the solicitation, for the period November 1, 2004, through October 31, 2005, with option to extend four additional years. Service will originate and terminate at the Orlando VA Healthcare Center, Orlando, FL. Note No 29 applies. Subject to the availability of funds. All quotations are to be submitted by hard copy to the designated office by COB October 19, 2004. Faxed versions are acceptable. All other electronic versions are not acceptable. Electronic version and hard copy of solicitation will be available October 6, 2004. Electronic request for quotation may be obtained free at the following URL:http://www.fedbizopps.gov . Any amendments or additional information issued to this request for quotation will also be available electronically at the URL address. Request for hard copy solicitation must be in writing. No telephone request will be accepted. Only written or faxed requests received directly from the requestor, no later than October 13, 2004, are acceptable.
