Description
NAICS Code: 562211 This is AMENDMENT #1 TO RFQ-RT-04-00504. The due date for receipt of proposals has been changed from October 12, 2004 to October 27, 2004 at 12:00 noon. Amendment #2 to address questions from interested parties will be issued soon. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL SERVICES PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. This announcement is a Request for Quotation (RFQ). This procurement is being conducted under Simplified Acquisition Procedures in accordance with FAR Part 13. This solicitation document and incorporated provisions and clauses are those in effect through the September 2001 Federal Acquisition Regulation revision, which includes the consolidation of all Federal Acquisition Circulars through 97-27. This acquisition is 100% set aside for small business concerns and the North American Industry Classification System (NAICS) Code is 562211 "Hazardous Waste Treatment and Disposal" with a size standard of $10.5 million in average annual revenue over the preceding three fiscal years. EPA anticipates the award of a fixed price purchase order with a one year period of performance. This procurement is for the services of a radioactive waste management contractor to classify, inventory, package, label, mark, load, transport, store, treat and dispose of hazardous/radioactive waste. The contractor shall be responsible for, but not limited to, all services, equipment, labor, materials, supplies, fees, packaging, transportation, storage and disposal of hazardous/radioactive waste. Prospective contractors or subcontractors shall have current applicable licences and permits to perform these tasks and, must be able to comply with all applicable federal, state, and local laws, ordinances, and regulations governing handling, transportation, and disposal of hazardous/radioactive waste materials and environmental protection. All offerors must submit a detailed technical proposal which must include: demonstrated knowledge, qualifications, capabilities, and experience in hazardous and radioactive waste disposal of the contractor, and any of its subsidiaries or subcontractors who will be performing under this contract; qualifications, experience, and capabilities of the radioactive and mixed waste treatment, storage, and disposal facilities or other facilities handling AED radioactive waste and, qualifications, experience, and capabilities of the radioactive and mixed waste transportation component performing work under this contract. The following FAR provisions apply to this solicitation: 52-212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items. Evaluation criteria to be included in paragraph (a) of provision 52.212-2 are as follows: 1) Technical Capability - Proposals must demonstrate ability of the contractor to meet requirements set forth in the Statement of Work and in accordance with Evaluation Criteria 1, 2, and 3. Proposals must include sufficient material(such as narrative of technical ability, publications of work, brochures, etc.) to permit the government to determine that the contractor can meet the requirements set forth in the Statement of Work. Any specific variances from the Statement of Work shall be addressed by the offeror with respect to how the offeror's proposal provides alternative equivalence. Failure of an offeror to demonstrate that proposed alternate methodology will meet the Statement of Work requirements shall render the offeror ineligible for further consideration for award. 2) Past Performance - Proposals shall be evaluated on performance under existing and prior (within past 3 years) contracts/subcontracts for work similar to that described in the Statement of Work. The Government's evaluation shall focus on the quality of past performance and level of customer satisfaction. For each contract/subcontract identified, the offeror shall include a brief synopsis which includes the date the work was performed, the client for whom the work was performed (included client name and telephone number), and a description of the work performed. 3) Price. Award will be made to the responsible offeror whose offer is determined to be the best overall value to the government, price and other factors considered. For the purpose of this best value evaluation, all evaluation factors other than cost or price when combined are significantly more important than cost or price. A single award will be made to the offeror that represents the best value to the Government. All offerors are to include with their offer a completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items.
