- The Contractor shall furnish all personnel, material, services and facilities to provide and maintain the high speed access service and hardware requirements set forth in the Statement of Objective (SOO). Provide a price for nationwide flat rate/always-on high speed digital Internet access. Minimum speed shall be 512 kbps download and 128 kbps upload. The services include the provision of high-speed digital Internet access to current Federal Occupational Health (FOH) remote offices as well as allow for growth over the life of the contract. The Government requirement will be divided into 3 phases 1) implementation of FOH selected pilot sites [approximately 10 sites] 2) implementation of the remaining sites [approximately 300 nationally dispersed locations] and 3) ongoing moves/adds/changes. Upon successful implementation of the pilot sites (phase 1), the government will begin implementing the remaining locations. The Contractor shall manage the delivery of service through a phased implementation approach for the 300 FOH sites. Implementation schedule will be jointly determined by the government and Contractor and will likely be on a geographic basis. The Contractor shall provide overall project management support and serve as the single point-of-contact to the Government for implementation and acceptance of the pilot sites and each of the 300 sites. Upon completion of the existing sites (see ?List of FOH sites?), the Contractor shall perform ongoing moves/adds/changes through normal business processes and will provide consolidated monthly billing for the FOH sites. The Government will order services under this contract by means of Delivery Orders (DO) or a similar document with specifically defined scope, product, and services. All Delivery Orders (DO) issued and awarded under this contract shall be priced at rates no higher than those quoted by the Contractor. Such prices shall remain valid for the duration of the DO. Local loop pricing for DSL service may be provided on a site by site basis. Additional Delivery orders will be issued upon successful implementation of phase 1 sites. High speed digital access service options (i.e. DSL, Broadband Cable, Satellite) shall be provided on a fixed installation fee, and a fixed monthly service fee after implementation of the phase 1 and phase 2 sites. The following technical factors shall be used to evaluate quotes: Factor 1: Capability Evaluation: Capability will be evaluated in terms of demonstrated capability to provide DSL to as many of the FOH sites as listed in Attachment 3. Factor 2: Technical Summary and Corporate Experience: The offeror?s proposal demonstrates that the offeror possesses the capability to perform by possessing the managerial expertise to provide project management, implementation plan and the ability to maintain service levels. Project management will include all resources, including personnel and equipment to perform in accordance with the statement of objectives. An offeror?s proposal demonstrates experience providing relevant services by describing the tasks or projects undertaken over the past 5 years that are within scope of the statement of objective and of similar size and complexity. Factor 3: Past Performance: An offeror?s past performance will be evaluated with emphasis on how the offeror performed on efforts similar in scope to those in the statement of objective. Such performance, over the last 5 years, will be evaluated as an indicator of the offeror?s ability to satisfactorily accomplish the efforts proposed. Factor 4: Price The Government desires all sites to be implemented no later than July 31, 2005 after contract award.
