- This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The Request for Quotation number is HSCG89-05-Q-JDM001 and incorporates provisions and clauses in effect through Federal Acquisition Circular 01-25. This acquisition is unrestricted. The NAICS is 713940. The small business sized standard is $6 Mil Annually. The contract type will be a Firm Fixed Price Purchase Order. The Government proposes to solicit quotes for the following: Annual health club membership for all active duty Coast Guard military and civilian employees of the Coast Guard Seventeenth District, Juneau, Alaska (approx. 140 members). Facility must be within the geographic area of Juneau, Alaska. Fitness center membership shall include use of cardiovascular equipment, free weights, free participation in group fitness classes and racquetball courts. The vendor shall submit invoices monthly in arrears and shall only be paid for the actual number of active memberships during the billing period. AWARD: This purchase will be based on the following criteria: Lowest price for items. PERFORMANCE PERIOD: 01 Nov 2004 ? 30 Sep 2005 CLAUSES: The following FAR provisions apply to this acquisition: FAR 52.212-1 Instruction to Offers Commercial Items, FAR 52.212-3 Offeror Representatives and Certifications and Certifications Commercial Items, all offerors are to include with their offer a completed copy of FAR 52.212-3, FAR 52.212-4, Contract Terms and Conditions Commercial Items.
