PRE-SOLICITATION OUTLINE OF REQUIREMENTS: CLEANING, MAINTENANCE AND REPAIR KITCHEN EXHAUST AND WET/DRY CHEMICAL FIRE SUPPRESSION SYSTEMS. *********************************************** LENGTH OF CONTRACT: BASIC YEAR 12-1-04 THROUGH 9-30-05 PLUS FOUR OPTION YEARS ************************************************ LOCATION OF PERFORMANCE: ANDREWS AFB,MD & DAVIDSONVILLE AF SITE BUILDING LOCATION: 1228, 1287, 1442, 1628, 1658, 1672, 1778, 1810, 1889, 3763, 3464, 3725, 4442, 4575, 4700, 4783, 5016 AND BUILDING #1 DAVIDSONVILLE, MD ************************************************* BASIC SERVICES: 1.-REVIEW RECORDS- 2.-INSPECT/CLEAN KITCHEN HOOD, DUCT & FAN EXHAUST SYSTEMS- 3.-OPERATIONAL TEST AND MAINTENANCE ON SUPPRESSION SYSTEMS- 4.-RECURRING PREVENTIVE MAINTENANCE ON KITCHEN EXHAUST SYSTEMS- 5.-CORRECTIVE/REPAIR MAINTENANCE ON KITCHEN HOOD EXHAUST SYSTEMS- 6.-EMERGENCY REPAIR- 7.-PERFORM IN ACCORDANCE WITH SAFETY STANDARDS AND APPLICABLE CODES- 8.-SYSTEM TEST, INSPECTION, AND MAINTENANCE REPORTS.- 9.-ALL PERFORMANCE OF CONTRACT WILL BE IN ACCORDANCE WITH NFPA STANDARD 96 AND OTHER LOCAL, STATE, FEDERAL AND U.S. AIR FORCE RULES AND PROCEDURES.- ************************************************************** REIMBURSABLE SERVICES: 1.-INSTALLATION/UPDATE NEW KITCHEN HOOD DUCT AND SUPPRESSION EQUIPMENT- 2.-REIMBURSABLE PARTS- 3.-SERVICE DELIVERY SUMMARY ? (SEE STATEMENT OF WORK FOR DETAILS) A.(PERFORMANCE OBJECTIVES) B.(PERFORMANCE THRESHOLDS) C.(METHOD OF SURVEILLANCES) **************************************************************** -GOVERNMENT FURNISHED PROPERTY AND SERVICES- WHAT THE GOVERNMENT WILL PROVIDE (SEE STATEMENT OF WORK) WHAT THE CONTRACTOR WILL PROVIDE (SEE STATEMENT OF WORK) **************************************************************** (GENERAL INFORMATION:) (QUALITY CONTROL :) -Contractor shall develop and maintain a quality control program to ensure maintenance and repair service are performed in accordance with applicable standards and the manufacturer?s instructions. The contractor shall develop and implement procedures to identify and prevent defective services from reoccurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in paragraph 2, Service Delivery Summary. The government evaluator must have a specific quality control inspector to notify in case of customer complaints.- *************************************************************** (QUALITY ASSURANCE :) -The government will periodically evaluate the contractor?s performance by appointing a representative (QA) to monitor performance through intermittent on-site inspections of the contractor?s quality control program and receipt of complaints from base personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated customer complaints. Likewise the government may decrease the number of quality control inspections if performance dictates. The government will also receive and investigate complaints from various customer located on the installation. The government representative shall make final determination of the validity of customer complaint(s) in case of disagreement with customer(s).- ***************************************************************** (DAMAGE :) -Contractor shall repair or replace any government property damaged by the contractor at no additional expense to the government.- ************************************************************** (WORK HOURS:) -Work hours (when possible) shall be during normal duty hours (0730 to 1630, Monday thru Friday, excluding Federal Holidays). Contractor shall coordinate with QAE to ensure QAE is present during all work. Work is usually performed after duty hours.- *************************************************************** (EMERGENCY:) -In the case of an emergency, the QAE, Base Fire Chief, and Base Operations Officer, or their representative, shall have the authority to order the contractor to terminate work and clear the area of personnel and equipment.- ***************************************************************** (BASE CIVIL ENGINEERING WORK CLEARANCE REQUEST, AF FORM 103.:) -If necessary due to the nature of work and prior to commencing work, the Contractor performing work shall be responsible for obtaining and having in their possession at all times while digging, an approved AF Form 103, in accordance with Andrews AFB Regulation 91-5. The Contractor may contact 89th Civil Engineer Squadron Customer Service, Bldg 3465 North Carolina Ave, (301) 981-3010, to obtain a digging permit package. The Contractor SHALL PROVIDE a Miss Utility ticket number to the CO or his/her representative at least 24 hours prior to digging.- **************************************************************** (Security:) *The contractor shall conform to all Base Regulations and directives that pertain to security, safety, traffic, fire and personnel clearances. To obtain passes for all work on Andrews AFB, the contractor shall submit the following information on each employee no later than 10 working days before expected arrival on base to 89 CES/CEOE at (301) 981-2055: QAE POC: MR. DAVE FORSHT 301-981-6443 * a. (Full Name of Individual) b. (Social Security Number) c. (Company Name) d. (Vehicle Driver?s License State and Number) e. (Vehicle State and Tag Number) f. (Project individual is working) g. (Your Vehicle Registration) *Contractor personnel will then have to report to the Visitor?s Center, Bldg 1840, to obtain passes. The contractor shall be responsible for the return of passes issued to employees who separate from the contract before the expiration date of each contract period including any extensions to the contract. Lost or stolen passes shall be reported immediately to the contracting officer with a written follow up to the contracting officer within 72 hours.* ***************************************************************** (Health, Safety, and Environment:)-Contractor shall follow all applicable state, federal, and local laws pertaining to health, safety, and environment.- ***************************************************************** (Disposal:) -Contractor shall dispose of all excess material off site in accordance with applicable federal, state, and local laws.- ***************************************************************** (Period of Performance:) -Work shall be completed within 30 business days of contract award date.- ***************************************************************** (WARRANTY.) -The contractor shall be responsible for replacing any warranty/ guarantee items for (1 YEAR) after installation.- ***************************************************************** *GOVERNMENT REMEDIES.* (SEE DETAILS IN THE STATEMENT OF WORK) *APPENDICES.* (SEE DETAILS IN THE STATEMENT OF WORK)
