- FISC Norfolk Detachment Philadelphia intends to procure Phase I of a Family-Centered Care Program for the Bureau of Medicine & Surgery, Washington, DC. Request for Quotation (RFQ) N00140-05-Q-M007 completes a firm fixed price contract. Delivery of services is required within 10 days after receipt of order. This a combined synopsis/solicitation for commercial item prepared in accordance with Simplified Acquisition Procedures. This announcement constitutes the only solicitation; proposals being requested and a written solicitation will not be issued. It is the contractor’s responsibility to be familiar with the applicable clauses and provisions. The clauses may be access in full text at theses addresses: www.arnet.gov/far and www.dtic.mil/dfars. The NAICS Code is 512110. It is requested that qualified sources capable of providing services for line item numbers.(CLINs): CLIN 0001 PHASE I OF DEVELOPMENT OF A TRISERVICE VIDEO THAT WILL BE USED IN THE FAMILY-CENTER CARE PROGRAM, PHASE TO CONSIST OF ALL INPUT TO THE FINAL VIDEO; SCRIPT DEVELOPMENT, ANIMATION AND FILMING OF “B-ROLL” FOOTAGE AT NAVHOSP PENSACOLA, WALTER REED MEDICAL CENTER AND EGLIN AFB. THE CONTRACTOR WILL PROVIDE PRE-PRODUCTION SUPPORT SERVICES, TO INCLUDE SCRIPT DEVELOPMENT AND SCRIPTWRITING SUPPORT TO STORYBOARD VIDEO; PRODUCTION SUPPORT TO LAYOUT A PLAN FOR FILMING AND PRODUCING A VIDEO AT MULTIPLE LOCTIONS; TRAVEL TO THREE (3) CONSUS MTFs FOR LOCATION SURVEY, PREPRODUCTION INTERVIEWS AND B-ROLL VIDEO TAPING; PRODUCE THE B-ROLL SEQUENCES AND GRAPHIC FOR THE VIDEO, TO INCLUDE DIRECT AND VIDEOTAPE B-ROLL SEQUENCES FOR INCLUSION IN COMPLETED VIDEO WITH BOARDCAST QUALITY VIDEO PRODUCTION EQUIPMENT; PROVIDE ALL REQUIRED CREW AND EQUIPMENT FOR TAPING B-ROLL AT THREE MTFs LOCATED IN DIFFERENT CONUS LOCATIONS; CREATE COMPUTER 3D ANIMATION AND PHOTO REALISTIC ANIMATION SEQUENCES FOR INCLUSION IN COMPLETED VIDEO. THE GOVERNMENT SHALL PROVIDE ALL NECESSARY SUPPORT DOCUMENTATION AND POINTS OF CONTACT NECESSARY TO PERFORM THE WORK. MTFs WILL PROVIDE A SINGLE POINT OF CONTACT FOR ALL PROPOSED WORK SCOPE AND POINT OF CONTACT FOR EACH MTF. MTFs WILL ALSO BE RESPONSIBLE FOR: MEETING SPACE. ADMINISTRATIVE SUPPORT TO INCLUDE WORD PROCESSING ASSISTANCE, PHOTOCOPYING; NOTIFYING STAFF MEMBERS REGARDING ON-SITE VIDEOTAPING ACTIVITIES; IDENTIFYING STAFF MEMBERS AND PATIENTS FOR INCLUSION IN VIDEOTAPING; OBTAINING CONSENT FORMS FOR RELEASE OF VIDEO FOOTAGE (PUBLIC AFFAIRS/LEGAL OFFICE). NAVAL MEDICINE/DOD/MILITARY HEALTH SYSTEM WILL USE VIDEO FOR INTERNAL EDUCATION PURPOSES ONLY. TIME FRAME – ALL ACTIVITIES NECESSARY TO PREPARE IN THE FINAL SURVEY VIDEO WILL BE COORDINATED/DIRECTED BY BUMED AND COMPLETED WITHIN THE CONTRACT PERIOD. ALL DELIVERABLES SHALL BE PROVIDED IN ELECTRONIC AND/OR HARD COPY FORMAT. DELIVERABLES SHALL BE IN THE FORM OF HTML DOCUMENTS, MICROSOFT WORD, POWERPOINT OR ACROBAT PDF FORMAT. SCRIPT/STORYBOARD VIDEO (1 MASTER SET); PLAN FOR FILMING AND PRODUCING VIDEO AT MULTIPLE LOCATIONS (1 COPY); COMPUTER 3D ANIMATION AND PHOTO REALISTIC ANIMATION SEQUENCE FOR INCLUSIOINS IN COMPLETED VIDEO (1 MASTER SET); B-ROLL VIDEOTAPE (1 MASTER SET). INVOICES – THE ITEMIZED INVOICE SHALL REFLECT THE TOTAL CHARGES BEING BILLED. The provisions at 52.212-1, Instructions to Offerors Commercial Items is applicable to this procurement. IN DEPTH ON-CAMERA INTERVIEWS WILL BE CONDUCTED WITH ALL LEVELS OF HOSPITAL STAFF, PATIENTS AND FAMILY MEMBERS INVOLVED IN THE PROGRAM, THE GRAPHICALLY ILLUSTRATE THE NEED, STAKEHOLDER ACCEPTANCE AND OVERALL SUCCESS OF THE PROGRAM. CREATIVE APPROACH: THE PROGRAM SHOULD BE DESIGNED AS A MAGAZINE RATHER THAN A PROMOTIONAL DOCUMENTARY. THE PRODUCTION SHOULD BE CENTERED ON AN UNSEEN PROFESSIONAL HOST(S), WHO WILL NARRATE AND PROVIDE TRANSITIONS TO THE VARIOUS ILLUSTRATIVE SEGMENTS OF THE PROGRAM. THIS INDIVIDUAL SHOULD BE EXCELLENT HONED NARRATION AND PRESENTATION SKILLS. THE PRODUCTION SHOULD BE CREATIVE AND MOVING TO BE SPECIFICALLY DESIGNED TO GRAB AND HOLD THE AUDIENCE’S ATTENTION. MUSICAL SCORE: AN IDENTIFIABLE MUSICAL THEME IS DESIRED, SOMETHING THAT IS UPLIFTING AND ENERGIZING THE MAIN THEME OF PRODUCTION.
