Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation document. Request for Quotes (RFQ) for Floor maintenance are being requested for Outpatient Clinics of the South Texas Veterans Health Care System, and a written solicitation will not be issued. This notice incorporates provisions and clauses in effect through FAC 01-24. This solicitation is 100% set aside for Small Business. The associated NAICS Number is 561720. A Firm-Fixed Price contract is anticipated. The period of performance is from date of award to September 30, 2005 with a one-year renewal option. Line item one: The Frank Tejeda Outpatient Clinic (FTOPC), San Antonio, Texas, four jobs; Line item two: The Corpus Christi Outpatient Clinic (CCOPC) , Corpus Christi, Texas, four jobs, and Line item three: The McAllen Outpatient Clinic (MOPC), four jobs, require floor maintenance to be performed four times annually (Dust, Mop, buff & Shampoo three times per year.) Statement of work: I. In June, refinishing of hard floors and carpet cleaning to be provided. II. Floor maintenance to be provided three times (December, March, August ) shall consist of : A. Dry mopping and wet mopping of all hard floors, followed by wet scrub, rinse and application of a light coat of Johnson & Johnson Premia wax along with thoroughly drying and burnishing of all composite tile floors. B. Mop boards, kickboards, baseboards, walls and furniture shall be cleaned after floors are complete to remove any wax, dust, stains etc. C. Stairs in stairwells shall be scrubbed and a light coat of Johnson & Johnson Premia Wax shall be applied. All the landings on the stairwells shall also be scrubbed and a light coat of Johnson & Johnson Premia Wax shall be applied. D. All horizontal surfaces (rails, door frames, window sills, cabinet tops, counters etc) shall be cleaned. Only surfaces with paperwork on them shall be excluded. Window blinds or drapes shall be dusted and cleaned before cleaning of adjoining surfaces. E. All ceramic and metal fixtures (water fountains, sinks, commodes, and urinals) shall be scrubbed inside and exterior surface to remove any build up or stains and leave a clean and shiny appearance. F. Vinyl and plastic seating and supporting frames shall be cleaned of stains and any dirt or dust. Surfaces shall be left clean with no residue from cleaning. G. Terazzo, ceramic tile, quarry tile, and nonconductive flooring shall be dusted, wet mopped/scrubbed and buffed. Nonconductive floor cannot have any finish applied on it. H. Carpets shall be steam extracted followed by application of scotch guard. Carpets shall be extracted on Friday after the clinic is closed. III. Annual floor maintenance of all hard floors. A. Shall include stripping to remove all old finish and or sealer, and rinsed until neutralized in preparation for any application of finish or sealer. B. Next two coats of Johnson & Johnson Premia shall be applied with at least one hour of drying time between coats. Twenty-four hours later the floor shall be burnished, using a minimum of a 350 RPM buffer, thoroughly dust mopped and wet mopped before applying a third coat of the same wax. C. All ceramic tile, quarry tile, terrazo etc shall be stripped leaving the floor and grout uniform in color and clean. After neutralizing the floor two coats of an approved sealer (STVHCS currently uses Cornerstone by 3M) shall be applied after the floor has had adequate time to dry. At least one hour shall be given for drying time between applications of the first and second coats of sealer. All furniture shall be moved out of the way by the contractor leaving no unfinished areas. All items shall be returned to their original position. No wax shall be applied to baseboards. D. Nonconductive flooring shall be dusted, wet mopped and buffed as needed; no finish of any kind can be applied to this flooring. All area adjoining floors being refinished shall be protected from splash or runoff during the process. No mop strands, hair, streaks, swirl marks shall be visible in the final prepared floor. Mop boards, kickplates, furniture, baseboards, walls and furnishing shall be cleaned after the floors are completed to remove any splash marks, wax, or dust from burnishing that remains on them. E. All work performed in both phases shall be done between 5 PM and midnight Monday through Friday. Scheduling of dates for all phases of work under this contract shall be scheduled at least three months in advance. Each phase of the work shall be performed on a continuous basis until complete. Work areas must be left clean, safe, and ready for use on the following morning. Our representative prior to award of the contract shall approve any desired chemical substitution. Chemicals to be used to perform work under this contract must be submitted for approval, and MSDS must be provided, prior to their use within any of these facilities. F. Each site must be visited before work is initiated to ensure coordination, support and access. Approximate sizes of the CCOPC, 20,000 sq ft., address: 5283 Old Brownsville Rd., Corpus Christi, Texas; McAllen OPC, 25,000 sq. ft., address: 2101 S. Col Rowe Blvd., McAllen, Texas, and SATX OPC, 50,000 sq ft., 5788 Eckert Rd., San Antonio, TX. The following provision and clauses apply to this RFQ: 52.212-2, Evaluation - Commercial Items. Evaluation criteria: The RFQ includes the Commercial Items evaluation provision (52.212-2) which provides that award shall be made to the responsible contractor whose quote is most advantageous to the Government, price and other factors considered. The significant evaluation factors are i) price, ii) past performance, and iii) Technical Expertise. Technical Expertise is more important than Past Performance and Price. The contractor shall return a completed copy of this provision with its quotation. Provision 52.212-1, Instruction to Offeror--Commercial Items, provision 52.212-3, Offeror Representations and Certification--Commercial Items ( return completed copy of this provision with quotation.) The following VAAR clauses and provisions are added by addendum: 852.233-70, Protest Content; 852.233-71, Alternate Protest Procedure; 852.237-70, Contractor Responsibilities; 852.270-4, and Commercial Advertising; 852.270-1. PARTIES INTERESTED IN RESPONDING TO RFQ may submit their quote in accordance with standard commercial practice (i.e. quote form, letterhead, etc.) and MUST INCLUDE THE FOLLOWING INFORMATION: RFQ number, offeror's complete mailing and remittance addresses, discount terms; unit prices and extended total price for line itmes1 through 3; DUNS number; and COMPLETED FAR 52.212-3 Alt I, and three references for evaluation of past performance of same or similar services. As of October 1, 2003, all Contractors are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to <http://www.ccr.gov/>. Interest parties, in responding to RFQ may submit their quote in accordance with standard commercial practice (i.e.quot3 form, letterhead, etc.) and must include RFQ number, offeror's complete mailing and remittance address; discount terms; unit prices and extended total price for line items. The Contractors are reminded that they are responsible to confirm receipt of quote and of all amendments. The Government intends to evaluate offers and make contract award without discussion.
