Description

The National Cemetery Administration is soliciting quotes (RFQ 786- 25-05) for concrete grave liners, furnished, delivered, and installed, perform pre-placed crypts and 2nd interment casket burials F.O.B. Destination within Contractor?s premises, Abraham Lincoln National Cemetery, 27034 South Diagonal Road, Elwood, IL 60421. Contract will cover Base period (date of contract award), through September 30, 2005, with up to four (4) one (1) year options to extend the term of the contract through September 30, 2009. This acquisition is 100 percent set-aside for small business concerns. North American Industrial Classification System (NAICS) Code 327390 and a size standard of 500 employees are applicable. All responsible small business sources may submit a quote. Request for a hard copy of the solicitation must be in writing to Mrs. Janice Corlew, Contract Specialist, and accompanied by a check or money order in the amount of Twenty-Five Dollars ($25.00) (non-refundable), payable to ?U.S. Department of Veterans Affairs.? Please reference Solicitation Number RFQ 786-25-05 on remittances. No telephone requests will be accepted. Hard copy solicitations will be issued via the ordinary mails on or about November 01, 2004. The solicitation may also be obtained FREE OF CHARGE, from VA?s Business Opportunities System via the Internet at the following URL address on or about November 01, 2004: http://www.bos.oamm.va.gov/solicitation?number=786-25-05. Any amendments to the solicitation will also be available at the URL address. Quoters are advised that they are responsible for obtaining amendments. CBD Numbered Note 1 applies.
