Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is set-aside for small business only. The NAICS Code for this synopsis/solicitation is 621340, Size Standard $6,000,000. Solicitation/Purchase Request number FOX51N42580100 is issued as a Request for Quote (RFQ). This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2001-5 effective 05 October 2004. The Government intends to award one Contract Line Item Number (CLIN 0001) for the work described in the below Statement of Work (SOW); QUANTITY: 1 YR. DESIRED PERIOD OF PERFORMANCE: 15 Nov 2004 - 30 Oct 2005 FOB: DESTINATION. Inspection and Acceptance: DESTINATION. The following provisions and clauses apply to this procurement: FAR 52.212-1, Instructions to Offerors-Commercial Items; In accordance with the FAR 13.106-1(a)(2) offerors are notified that award will be made on the basis of price and past performance. Offerors are required to submit at least three references for past performance. Price and past performance are of equal importance. Offeror will be evaluated in accordance with FAR 13.106-2. GENERAL PROVISIONS LIST OF FAR CLAUSES INCORPORATED BY REFERENCE The provisions of the following clauses set forth in the Federal Acquisition Regulation (FAR) or DoD FAR Supplement are hereby incorporated into this order or contract by reference with the same force and effect as though herein set forth in full. As used in the following clauses, the term "Government" is deleted and the abbreviation "NAFI" is substituted in lieu thereof. The date of each clause shall be the current date set forth in FAR or DoD FAR Supplement on the issuance date of this order or contract. Clauses made inapplicable by the reference or by the kind of order, contract (e.g., orders or contract for services instead of supplies) or dollar amount are self-deleting. The complete text of any clause incorporated in this order or contract by reference may be obtained from the contracting officer. CLAUSE NO. This is a nonappropriated fund purchase and it does not obligate appropriated funds of the United States Government. Nonappropriated funds are generated by the military community through the sale of goods and services and the collection of fees and charges for participation in military community programs. This purchase does not involve federal tax dollars. Statement of Work 1. The contractor will provide massage therapy to military, DoD, Contract personnel, and their eligible family members at the Hanscom AFB Fitness Center, or another site as coordinated between the Fitness Center Director (FCD) and the contractor. 2. For basic massage therapy. The increments of time per massage will be any one of the following: ? hour, 1 hour, 1 ? hours, 2 hours, or chair massages at 1 min increments. 3. Contractor must be willing to work Monday through Friday and/or weekends, and to accept appointments before and after normal military duty hours. 4. Appointments will be set by the contractor and agreed upon by the client. Special workshops may be set up as agreed upon by the contractor and the FCD. 5. If the contractor cannot provide service due to leave, training, inclement weather, or a family emergency, he/she will be responsible for giving sufficient notice to the client(s) and the FCD about appointment cancellation. 6. The FCD shall furnish room, storage area, and cash register. The contractor shall furnish all other supplies. The contractor shall be responsible for returning room to the same condition under which it was found and for complying with all applicable publications. 7. All fees will be collected by the Fitness Center. The Fitness Center staff shall keep a log of all fees collected throughout the month. During the last week of each month, the contractor shall submit to the FCD a recapitulation of all clients receiving massage therapy service. The FCD and the contractor shall compare and reconcile all receipts to make sure that both parties agree on the amount of fees collected for the month. Payment basis to the contractor is 90 percent of each receipt collected. Payment of the remaining 10 percent shall be made to the Non-Appropriated Fund Instrumentality (NAFI). 8. The contractor shall submit proof of a massage therapist license OR have completed 500 hours of study and passed either a state certification or the National Certification Examination for Therapeutic Massage and Bodywork (NCETMB). 9. The contractor shall have extensive knowledge of and practice in sports massage therapy. 10. Failure to meet any of the above requirements may result in termination of this contract. See Note 26.
