- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4620-05-Q-B001 is issued as a request for quote, utilizing Simplified Acquisition Procedures in Federal Acquisition Regulation (FAR) Part 12 and 13. Resultant contract will be firm-fixed price. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2001-25. NAICS Code is 561710. Small business size standard is $6M. This acquisition is 100% Small Business Set-Aside. Fairchild AFB has a requirement for Professional Airfield Bird Control Services for mitigating risk of bird strikes in accordance with the Statement of Work. The contract will cover the period 1 Feb 05 thru 30 Sep 05 with four, one-year option periods. Submit the following items with the quotations: 1) Pricing Schedule, 2) Past Performance information, to include recent and relevant contracts for the same or similar services and other references (including contract numbers, points of contact with telephone numbers and other relevant information), 3) Master of Falconry License, and 4) Completion of on-line Representations and Certifications. Quotes are due by 19 Nov 2004 no later than 4:30 P.M. Anticipated award date is 16 Dec 2004. Commercial Items (Jan 2004) Addendum to 52.212-1: Past Performance Information. The Government will evaluate the quality and extent of offeror?s experience deemed relevant to requirements of the SOW. The Government will use information submitted by the offeror and other sources, such as other Federal Government offices and commercial sources to assess experience. Provide a list of no more than five relevant contracts performed for Federal agencies and commercial customers within the last three years. Relevant contracts include those for airfield bird strike mitigation of the same scope, complexity and magnitude as required under this solicitation. Furnish the following information for each source of past peformance information: Company Name/Service provided/Contract Agency/Contract Number and Dollar Value/Performance Period/Verified, current name, address, FAX and telephone number of the contracting officer/Comments regarding compliance with contract terms and conditions and details regarding any known unacceptable performance with explanation of the resolution. The quote acceptance period (See Para ?c)?) is 60 calendar days. Evaluation--The Government will award a contract resulting from this synopsis/solicitation to the responsible offeror whose offer conforming to the solicitation will provide the best value to the Government, price and other factors considered.
