Description
As requested by Shawano County WI FSA Office for work in Outagamie and Shawano Counties Wisconsin. 104 of each task may be required although not all borrowers and/or all contracts (see SF-18!) will require all actions. STATEMENT OF WORK/SPECIFICATIONS (AGAR 452.211-72) (FEB 1988) The Contractor shall furnish the necessary personnel, material, equipment, services and facilities (except as otherwise specified), to perform the Statement of Work/Specifications referenced in Section C and J. ATTACHMENTS TO STATEMENT OF WORK/SPECIFICATIONS (AGAR 452.211-73) (FEB 1988) The attachments to the Statement of Work/Specifications listed in Section J are hereby made part of this solicitation and any resultant contract. C.1 Background. Farm Service Agency (FSA) requires the services of qualified persons or firms to provide FSA Farm Loan Programs the following services beginning January 3, 2005: January 1, 2005 balance sheet information from specified FSA borrowers (Farm Business Plan Worksheet, Form FSA-2037). 2004 actual income and expense data on specific farm borrowers Performance of farm servicing visits for specified farm borrowers. NOTE: At no time will FSA facilities, telephones or equipment be used by the contractor in the performance of this contract. C.2 General. For questions on the statement of work, please email ray.ellenberger@wi.usda.gov. Upon receipt of a delivery order from the Ordering Official under this contract, the contractor shall arrange farm appointments with the FSA borrowers, and follow the specifications listed below for each assignment listed in the delivery order. C.3 Statement of work. 1. Task 001 ?Farm Business Plan Worksheet (Balance Sheet),? Form FSA-2037 (See Exhibit A): a) The contractor will meet with the FSA farm borrower and complete Form FSA-2037, ?Farm Business Plan Worksheet (Balance Sheet)? (Exhibit A). This form will be completed showing information as of January 1, 2005 and will be completed in detail (pages 1 - 4) based upon a meeting with the borrower. b) The borrower and co-applicant?s signature will be obtained and the form dated. c) The contractor will provide a short summary of the meeting with the farm borrower detailing the date they met with that borrower and who the contractor?s representative was who obtained the information. This information may be entered in item 9E, page 3 of the Exhibit A. 2. Task 002 (Farm Business Plan Worksheet) Actual 2004 Income and Expense (See Exhibit B): a) The contractor will meet with the FSA farm borrower and complete Exhibit B by reviewing the monthly financial record entries used to generate the annual summary of the family's financial record-keeping system. That review will be made to assess whether the information is up to date and accurate. b) The contractor will obtain the borrower and co-applicant?s signatures in Part E of Exhibit B. c) The contractor will attach a summary of the meeting with the borrower(s) which will include as a minimum the date of the meeting(s), the name of the contractor?s representative who met with the borrowers, the persons the contractor met with and any facts on production, payments made, decrease or increase in debts, borrower needs and observations made by the contractor that should be considered in any future loan making assessments. 3. Task 003 Farm Visit Summary (See Exhibit C): The contractor will, within 30 days of the receipt of the task order, meet with the borrower(s) for a visit at the farm operation site. During the visit the following information will be obtained and/or discussions conducted with the resulting information provided to FSA on ?Farm Visit Summary? (Exhibit C) which includes: a. Record overall condition of farm site. b. Inventory feed supply (based on interview with client) - enter description and note if supply short or in surplus for the year c. Discuss crop rotation (if applicable): d. type of rotation planned or implemented, etc. e. Note general condition of crops (if applicable) ? relative to neighboring fields. f. Record specific agronomic concerns (if applicable), such as: 1. Plant population 2. Disease and insect control 3. Weed control 4. Soil testing g. Verify income and expense records are up to date. h. View livestock program records. i. Review of DHIA or other Production Testing Records, and if no records. j. Review of Reproductive Records, and if no records. k. View all livestock ? all locations AND general quantities l. View machinery (note general condition and obvious discrepancies) m. Obtain listing of capital purchases since last Security Agreement n. Review of Feeding Program. o. Obtain listings of: 1. Open accounts that are past due ? as of date of visit 2. New loans since last balance sheet taken by FSA. p. Complete Exhibit K record conclusions and summary of farm visit. q. Note any concerns that the borrower(s) had or that are observed that should be followed up on by an FSA loan officer.
