Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number USSS05Q0003 is issued as a Request for Quotation (RFQ) through Federal Acquisition Circular 01-25. This action is a 100% small business set-aside. The NAICS code is 713940, Fitness and Recreation Sports Centers and the size standard is $6 million. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-25. THIS ANNOUNCEMENT AND THE DOCUMENT IN THE ELECTRONIC POSTING SYSTEM (EPS) ENTITLED, "STATEMENT OF WORK FOR FITNESS COORDINATOR TRAINING" CONSTITUTE THE ONLY SOLICITATION; A WRITTEN SOLICITATION WILL NOT BE ISSUED (FAR 12.603(2)(i)). The United States Secret Service has a requirement for two physical fitness instruction, training and certification courses. Solicitation Number USS05Q0003 is issued as a Request for Quotations (RFQ). The offeror shall review the Statement of Work (Attachment A) and present a quotation as follows: PART A - TECHNICAL PROPOSAL -- The offeror shall provide fitness coordinator training and certification to U.S. Secret Service and Uniform Service Personnel, as well as administer physical fitness tests, and provide strength training and aerobic conditioning programs. The maximum number of personnel for the each course shall not exceed forty per class. The training classes will be held twice a year. The government reserves the right to postpone, reschedule or cancel training for the benefit of the USSS. The training course shall include physical screening, fitness testing and nutrition assessment as defined in the scope of work. Additionally, the course must provide study guides that include current industry guidelines, protocols, standards, exam review and a comprehensive exam for certification. The technical proposal must include a sample training packet for evaluation, along with points of contact by name, telephone number, address, and organization for which the lesson plan and course booklet was used in an actual training course. All proposals should include documentation showing how their training course materials specifically meet the technical requirements of the Secret Service. PART B - PRICING -- Prices shall be provided as follows: Line Item 0001 -- Fitness Coordinator Training Courses, 2 each; Line Item 0002 ? Nutrition and Exercise Courses, 2 each; Line Item 0003 ? Online Fitness Coordinator Review Course; Line Item 0004, Fitness Coordinator Training Courses, 2 each; and Item 0004 (Option) -- Fitness Coordinator Re-certification Course; Line Item 0005 ? Option Year One; Line Item 0006 ? Option Year Two. The Government intends to award a fixed price contract to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. An award may be made to other than the lowest-priced proposal received. It is anticipated that this requirement will be a fixed price contract for one base year and two option years. All responsible small business sources may submit a quotation which will be considered by the U.S. Secret Service (USSS). For evaluation of past performance, the offeror must provide at least three references for contracts/purchase orders of similar services within the last two (2) years. Each reference must include the customer's name and address, contact person, project name, date of performance, telephone number and contract value for each reference. If business entity establishment is less than two (2) years, provide related experience of key personnel. A signed copy of the offerors Representations and Certifications shall be included with proposal. QUOTATIONS ARE DUE ON FRIDAY, DECEMBER 10, 2004, 2:00 P.M. EST. Quotations must include all materials referenced herein, and may be sent by overnight mail via Federal Express to U.S. Secret Service, Procurement Division, 950 H Street, N.W, Washington, DC 20223, Attn: Donita Hood. If quotations are sent by United States Postal Service, they may be delayed due to mail inspection or screening, please allow adequate time for delivery before due date. If offers are hand delivered, please ensure that the courier is instructed to use the courtesy phone in the right rear of the lobby to call Procurement Operations on extension 6-6790/6940. A staff member will meet the courier to receive the submittal.
