Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E050001, is issued as a Request for Quotes (RFQ) under test procedures of FAR 13.5. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 621340 and the business size maximum is $5Million. The proposed contract is set aside for small businesses. This office is requesting quotes on the following items: 0001 Base Year Initial Evaluations, Psycho-Educational Assessment Services, QUANTITY is Ten, UNIT is Each. 0001A RE-Evaluations, QUANTITY is Eight, UNIT is Each. 0001B Consultation, QUANTITY is 22, UNIT is Days (8 hour days). 0001C Travel, QUANTITY is One, UNIT is Lump Sum.. This line item includes 22 nights for lodging and mileage costs to travel to and from the school. These costs shall be reimbursed in accordance with the Federal Travel Regulations. QUANTITY is Ten, UNIT is Each. 0005A RE-Evaluations, QUANTITY is Eight, UNIT is Each. 0005B Consultation, QUANTITY is 22, UNIT is Days (8 hour days). 0005C Travel, QUANTITY is One, UNIT is Lump Sum.. This line item includes 22 nights for lodging and mileage costs to travel to and from the school. These costs shall be reimbursed in accordance with the Federal Travel Regulations. QUANTITY is One, UNIT is Lump Sum. The Psycho-Educational Assessment Services provided to students at Dennehotso Boarding School, Dennehotso, Arizona shall be performed in accordance with the following SCOPE OF WORK: Psycho-Educational Testing and Assessment. The contractor shall provide Psycho-Educational Assessment Services at Dennehotso Boarding School, East Highway 160, Dennehotso, Arizona 86535 to students who are identified to receive this service. Students receiving this service will be disabled requiring evaluations or newly referred students who may be disabled. There are approximately eight (8) students that require RE-Evaluations and Ten (10) students that require initial evaluations. The school shall require twenty-two (22) days of service during the school year beginning November 23, 2004 through June 30, 2005, with four Option Year (OY) periods. The period of performance for the Option Years are as follows: OY1 (July 1, 2005 to June 30, 2006), OY2 (July 1, 2006 ? June 30, 2007), OY3 (July 1, 2007 to June 30, 2008) and OY4 (July 1, 2008 to June 30, 2009). The Psycho-Educational Assessment Services will be in accordance with P.L. 105-17 and 34 CFR, Part B, 25 CFR 45, #1-80 and Section 504 of the Rehabilitation Act of 1973, 29 USC 794. The individual Psycho-Educational Assessment Services shall include: 1. The administration of battery of tests chosen in collaboration with Special Education Department. 2. The scoring of all tests administered. 3. The interpretation of the test results. 4. The provision of a Comprehensive written Report describing the results of the assessment to the school within 30 days of the parent?s permission to evaluate. 5. Documentation that parents and staff receive a copy of the individualized report. 6. Will be available to participate in MDET/MDT/IEP meetings and interpret results of the testing for parents and staff within 30 days of the parent?s signed Permission to Evaluate. 7. Follow the Arizona Evaluation Requirements Format. Attend the contractor provider?s meeting with the Western Navajo Agency, Special Education. 9. Provide their current state certification to Western Navajo Agency, Special Education. The Individualized written Psycho-Educational Report shall include: 1. Basic student data as furnished by the referral packets at the Special Education Office, Dennehotso Boarding School. 2. Diagnostic statement categories listed in 34 CFR. 3. Reason for referral. 4. Background information: Educational Status and History, Health Assessment, and prior Evaluation History. 5. Behavioral observations. 6. Assessment of academic achievement. 7. Assessment of social behavior. 8. Assessment of perceptual abilities. 9. Assessment of emotional behavior. 10. Summary report. 11. Recommendations: Placement, Instruction, Social Medical, etc. The Individualized Written Psycho-Educational Report shall also include: 1. Wechsler Intelligence Scale of Children III. 2. Woodcock-Johnson Psycho-Educational Battery Achievement. 3. Wide Range Achievement Test or appropriate achievement tests. 4. Goodenough-Harris Drawing Test. 5. Developmental Test of Visual Motor VMI-A. 6. AADM-Adaptive Behavior Scale, when necessary, or other assessment as deemed necessary based on individual needs of the students. The Contractor shall: 1. Be available twenty-two days, eight (8) hours per day during the contract period. The school psychologist will arrange to meet with the MDT and IEP Committee for their annual review, re-evaluations, and other concerns. 2. Follow the Arizona Evaluation Requirements as stated in Section 300.352, Evaluation Procedures. 3. Attend all school IEP and MDT meetings to review documents and provide professional input concerning the service delivery for psychological evaluations at these meetings. Closing Date of Quote is November 29, 2004, 8:00am local time (Albuquerque, NM). Contractors may fax their quotes to (505) 248-6941. Please include your TAX ID Number, DUNS Number, and CAGE Code when submitting your quote. Companies that do not have a CAGE code by the closing date of the quote shall not be considered for award. Individuals that do not have a background clearance IAW clause by the closing date of the quote shall not be considered for award.
