Description

Contractor shall provide all labor and materials to repair (14) Olympus Endoscopes Models 130, 140, and 160 and shall include local courier service, and unlimited repairs and refurbishment for the VA Medical Center located at 50 Irving Street, NW, Washington, DC for the period of 12/1/04 ? 9/30/05 plus four (4) option years. This solicitation is being issued as a Request for Quotation (RFQ) number 688-38-05. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 2001-25. The NAIC code is 811310. All work to be performed in accordance with the Scope of Work and all responsible sources may submit an offer in accordance with the solicitation which may be obtained electronically on 11/23/04 at the following URL: http://www.fedbizopps.gov. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments. Awarded contract may be extended for four (4) additional option years in accordance with the clause at 52.217-9, Option to extend the term of the Contract, subject to the availability of funds). Bidders responding to this announcement shall submit a bid along with the information required in 52.212-1 and a completed copy of provision 52.212-3, 52.217-8 Option to Extend Services, and VAAR 852.219-70 Veteran Owned Small Business (Dec 1990) to VA Medical Center (688/90C), Room GE 100, 50 Irving Street, NW, Washington, DC 20422 by 12/3/04 at 2:00pm Local Time.
