- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E050005, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 61171 and the business size maximum is $5.0 Million. The proposed contract is set aside for small businesses. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 0001: Provide all travel, supplies, materials, and personnel to provide training for teachers of students with disabilities in nonviolent crisis intervention for the BIA, OIEP, SPA, in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. The Government intends to award a firm fixed price contract. The period of performance is from December 6, 2004 through December 5, 2005. The SCOPE OF WORK (SOW) is as follows: SCOPE OF WORK. BACKGROUND. An Interagency agreement has been negotiated between the Bureau of Indian Affairs (BIA), Office of Indian Education Programs (OIEP), and the National Center for Chronic Disease Prevention and Health Promotion (CDC) to provide training and resources in the implementation of the ?Circle of Life? K-6 and Middle School HIV/AIDS curriculum for BIA funded schools and dormitories. To accomplish the provisions of this agreement, the Center for School Improvement shall provide a solicitation to acquire contractual services for this purpose. PURPOSE. The purpose of this solicitation is to provide training in the ?Circle of Life HIV/AIDS? Curriculum to Bureau of Indian Affairs funded schools and dormitories that will enable the Bureau of Indian Affairs to achieve the following objectives. 1. reduce HIV/AIDS and STD risk behaviors identified in the Bureau of Indian Affairs? (BIA) 1994, 1997, 2000/01, and 2003 Youth Risk Behavior Surveys (YRBS) through the provision of culturally-relevant educational materials, teacher and other staff training and technical assistance for the implementation of the Circle of Life HIV/AIDS and STD curricula; 2. to engage in evaluation activities related to the Office of Indian Education Program?s (OIEP) HIV/AIDS and STD prevention education programs; 3. to coordinate and collaborate with State Education and Tribal Agencies and other organizations in addressing HIV/AIDS and STD prevention among American Indian and Alaska Native Youth. TERM OF CONTRACT: The term of this contract is for 365 Calendars Days from date of award, with one option year period (365 calendar days). The term may be extended by the execution of a yearly option to extend the contract for one year. The BIA will retain exclusive right to execute said option to extend the contract. Objective 1: To expand the existing cadre of trainers to facilitate training of school/dorm staff at the 184 BIA funded school/dorms. Task 1-1: The Contractor will develop a schedule to provide training in the implementation of the Circle of Life HIV/AIDS K-6 and Middle School Curriculum at the 22 Field Education Line Offices. The training will target at a minimum; A. The Field Education Specialists and School Reform Specialists at the Education Line Offices. B. One staff member from each BIA funded school and residential program. C. State and local education agencies in the respective states that the Field Education Line Offices are located in. D. Tribal service providers, School Board members, and other Tribal officials of tribes located in the respective Education Line Office service areas. The Education Line Offices are: 1. Billings 2.Cheyenne River 3.Chinle 4.Crow Creek/Lower Brule 5.Eastern Navajo 6.Fort Apache 7.Fort Defiance 8.Hopi 9.Minneapolis 10.Northern Pueblos 11.Oklahoma 12.Pima/Papago 13.Pine Ridge 14.Portland 15.Rosebud 16.Sacramento 17.Shiprock 18.South and Eastern States 19.Southern Pueblos 20.Standing Rock 21.Turtle Mountain 22. Task 1-2: the Center for School Improvement shall provide training materials, and materials to be taken back to the school/dorm sites to facilitate the next phase of training in the HIV/AIDS curriculum which will be training at the school/dorm sites. Objective 2: To provide training in the implementation of the Circle of Life HIV/AIDS curriculum at the 184 BIA funded schools/dorms. Task 2-1: The Contractor shall coordinate with the Field Education Specialist at the 23 Field Education Offices to insure that one day of training in the implementation of the Circle of Life HIV/AIDS curriculum is provided to each of the 184 BIA funded schools/dorms. Task 2-2: the Contractor shall coordinate with the Field Education Offices to ensure all schools/dorms implementing the Circle of Life HIV/AIDS curriculum have access to the materials needed for the classroom and dormitory implementation of the curriculum by managing such request for materials with the Center for School Improvement. Objective 3: To integrate the Circle of Life HIV/AIDS curriculum and knowledge/awareness of HIV/AIDS and other STDs into other programs administered at BIA funded schools/dorms. Task 3-1: The Contractor shall encourage the schools/dorms to integrate the Circle of Life HIV/AIDS curriculum into the Title?IV-A Safe and Drug Free Schools and Communities program applications at the 184 BIA funded schools/dorms. Task 3-2: The Contractor shall encourage the school/dorm to integrate the Circle of Life HIV/AIDS curriculum into the Title-IV-B 21st Century Community Learning Centers (21st CCLC) applications at the 54 BIA 21st CCLC funded sites. Task 3-3: The Contractor will coordinate with the Division of Residential Life offices to integrate the Circle of Life HIV/AIDS curriculum into the Intensive Residential Guidance and other programs at BIA funded Residential programs. Task 3-4: The Contractor will expand and develop additional public relations media, i.e. Posters, Book Markers, pens, Water Bottles, T-Shirts etc. and provide school/dorm and community awareness of the Health Issues associated with HIV/AIDS and other STDs. Students will be provided the opportunity by the contractor to develop art and other media to use for HIV/AIDS and other STD prevention purposes. Task 3-5: The Contractor will expand and develop additional public relations media developed to bring awareness of HIV/AIDS and other STD health risks to Indian tribes, school boards, and state and local education agencies upon their request. Objective 4: To evaluate the effectiveness of OIEP?s HIV/AIDS and STD prevention education programs. Task 4/1: The Contractor shall provide an ongoing evaluation of the training process at the Education Line Offices and the school/dorm sites. This component of the overall evaluation of the project will include? A. Numbers of Participants at each training B. Sign in sheets that will include the school/dorm or agency represented plus contact information C. Evaluations to be completed by the participants that will provide participants the opportunity to express quantitative as well as qualitative evaluation of the training D. A composite report that will reflect the participant?s as well as the trainers perception of the trainings. Task 4-2: The Contractor shall provide a bi-annual and an annual report/evaluation of the overall project activity. The report/evaluation shall be a narrative form and shall contain quantitative data as needed to support the contents of the report. Task 4-3: The Contractor shall assist the Center for School Improvement and the CDC in the evaluation of the Circle of Life HIV/AIDS curriculum at the school/dorm sites. This assistance will include but is not limited to? A. Coordination of training or with holding of training at specified sites for the purpose of establishing an Experimental and a Control group for the purpose of comparative analysis. B. Assistance in the administration of data collection instruments that will be identified by the CDC evaluation team. C. Assistance in the acquisition of data from instrumentation utilized by the BIA and the individual school/dorm sites, i.e., Youth Risk Behavior Survey, American Drug and Alcohol survey, Cultural Pride Survey. A. Assistance and coordination with contractors to be identified by CDC. B. Technical assistance to CDC in the implementation of evaluation strategies and processes. C. Assistance to CDC to insure that all requirements of research will be adhered to including: 1. Approval from appropriate Internal Review Board. 2. Adherence to Human Subjects protection criteria 3.Confidentiality when/where appropriate 4.Appropriate use of Research findings. TIMELINES: 1. The training process outlined in Objective 1, will commence at the signing of this contract and will be carried out on a yearly basis for the term of this contract. 2. The integration of The Circle of Life HIV/AIDS curriculum into the specified programs operated by the Center for School Improvement will be ongoing for the term of this contract. 3. The evaluation component will be ongoing for the term of this contract. 4. Bi-annual report for this contract will be delivered to the Center for School Improvement no later than the 15th of November each year of this contract. The Annual Report will be delivered to the Center for School Improvement no later than the 30th of May each year of this contract. REQUIRED QUALIFICATIONS OF THE EXPERT PROVIDER: The expert provider shall have a Bachelor?s Degree. The expert provider shall have four (4) years experience and knowledge in the health education field. The expert provider shall have two (2) years experience in the HIV/Health Education field, specifically working with minority communities. The expert provider shall have one (1) year experience in reporting Training Activities for Government Agencies. Packing and packaging shall be in accordance with good commercial practices.
