Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region Catheter & Tubing Securement Devices: Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Catheter & Tubing Securement Devices. Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing Catheter & Tubing Securement Devices. If your company currently holds, or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) for Catheter & Tubing Securement Devices, your company is eligible to participate in the standardization process. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than December 7, 2004. For questions, please call (619) 532-8517. 1.Does your company manufacture or distribute catheter & tubing securement devices for adult, pediatric and neonatal populations? If so, what types of catheters or tubing does your product secure? 2.Are your products available through the Prime Vendor, Owens & Minor? 3.What is your DAPA number? 4.Under what brand(s) does your company manufacture or distribute your product? 5.Under what standards is your product manufactured? 6.Are your products latex free? 7.Do you use any dyes in your products? 8.What material(s) is your product made of? 9.What type of adhesive does your company use for this product (self stick, pressure sensitive, etc)? 10.Is your adhesive non-allergenic? 11.Is your adhesive recommended for use with all skin types and populations? 12.Does your adhesive stay in place with various degrees of skin moisture (dry to diaphoretic)? 13.What is the recommended length of time this product should remain in place on the patient?s skin? 14.Can the patient shower/bathe with your product in place? 15.Does your company guarantee the patient will not experience discomfort with its use? 16.Does your product allow patient movement without dislodging or causing patient discomfort? 17.Does your product allow for visual inspection of the site at the skin? 18.Is your product sterile or non-sterile? 19.Is your product made of flexible material? 20.Does your product keep the catheter/tubing in place? If so, how? 21.Is your product reusable or single-use? 22.Is your product a single piece or multiple pieces? 23.Is the use and function of the product easily identifiable on the packaging? 24.Are there numbers and/or characters on the product? 25.Do your products have an accessible area for easy application of the patient ID label? 26.Do your products come in different sizes? If so, what sizes? 27.Do your products come in different colors? If so, what colors? 28.How many steps does it take to apply your product? Please list these steps? 29.Does this product require in-service training? If so, is this training provided by your company at no cost? 30.Does your company provide customer service 24/7? 31.Is there any history of backorders and/or recalls for this product group?
