See section "C" for statement of work.MILSTRIP: N0018305RQHCW11PURCHASE REQUEST NUMBER: N0018305RQHCW11 NET AMT FOB: Destination Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT POSITION SPECIFIC STATEMENT OF WORK FOR PARA PROFESSIONAL CASE MANAGEMENT ASSISTANTS (2.0 FTE) 1.0. SCOPE AND LOCATION. The contractor shall provide Case Management Assistant services for the support of Care Management Service Line for the Naval Medical Center, Portsmouth, VA (NMCP). Scope of duties includes direct patient services, consultation, and participation in Graduate Medical education programs, providing ongoing care for an enrolled population. The healthcare worker also contributes to the environment of care through maintenance of a safe and supportive environment for patients, staff, and other individuals served by or providing services within the organization. The healthcare worker will follow all standard operating procedures and will participate in educational and quality improvement programs within their assigned clinical area. Each healthcare worker will be assigned to a primary location; however, the Government retains the right to assign the healthcare worker to any Naval Medical Center, Portsmouth medical treatment facility within a 35-mile commuting radius of the NMCP proper. 2.0. DUTY HOURS. 2.1. The contractor shall provide healthcare workers who shall provide 8.5 to 9.0 hours of service (to include an uncompensated 30 or 60 minute meal break) between the hours of 0630 and 1630, Monday through Friday, not to exceed 80 hours of compensated service per 14-consecutive day period. At the mutual agreement of the healthcare worker(s) and the Government, alternative schedules may be implemented, such as a compressed work schedule; however, if the healthcare worker(s) agree(s) to a compressed work schedule, compensation will always be based on the applicable hourly rate for the position without compensation at an accelerated (overtime) rate. Specific hours and days shall be scheduled one month in advance by the workplace supervisor. Any changes in the schedule shall be coordinated between the healthcare worker and the Government. Healthcare workers shall always work through the completion of their clinical and administrative duties. The Government reserves the right to change shift hours with 72 hours notice to accommodate patient care needs. 2.2. Services are not required on the following federal holidays: New Year's Day, Martin Luther King's Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day. The Government will compensate the healthcare worker for these periods of excused absence. 2.3. For inclement weather the healthcare worker will follow the guidelines as established by the department by command instructions as directed by the Commanding Officer. 2.4. When required, to ensure completion of services that extend beyond the normal close of business, the healthcare worker will remain on duty in excess of the scheduled shift. The healthcare worker will be given an equal amount of compensatory time for hours worked that have been prospectively approved by the Government. Without prospective approval, there will be no compensatory time. The workplace supervisor and the healthcare worker will negotiate the use of accrued compensatory time. 2.5. Upon award, the Government will provide the Contractor with the scheduling template. For purposes of initial schedule submission, the Contractor may submit the first schedule for all personnel up to two weeks prior to commencement of services. All subsequent schedule templates shall be provided to the Contractor 75 days in advance of services and shall be returned to the COR 60 days in advance of services. The Government recognizes the unforeseen need for scheduling changes for events such as illness or injury. The Government for such occasions on a case-by-case basis may waive the required 72-hour advance notification for schedule changes. 2.6. Without the prospective approval of the Contracting Officer, no more than 1 individual shall be permitted to provide services for each work year (either 2,088 or 2,096 work hours per year) identified by the Government. 2.7. Each healthcare worker shall possess a valid driver?s license and shall provide his or her own transportation when Government transportation is unavailable. When using a personal vehicle for the duties contained herein, the healthcare worker shall be compensated for mileage at the prevailing rate. Healthcare workers shall not transport patients or members of patient?s families in either their personal or government vehicle without prior approval from the workplace supervisor. 3.0. LEAVE AND ABSENCES. The healthcare worker shall accrue 8.0 hours of combined sick/annual leave at the end of every 80 hours worked. The Government will compensate the Contractor for these periods of planned absence. The Contractor is not required to provide a qualified replacement if the healthcare worker has been approved to use accrued leave. 4.0. SPECIFIC DUTIES AND RESPONSIBILITIES. Healthcare workers shall perform in accordance with established principles, practices, ethics, and written policies, procedures and requirements of Naval Medical Center Portsmouth and shall apply age specific knowledge and competency appropriate to patient population served. The standards of performance for all healthcare workers shall reflect the degree of care, skill, and learning expected of a reasonably prudent healthcare worker in the category to which he or she belongs. 4.1 Case Management Assistant Duties and Responsibilities. Duties and responsibilities include, but are not limited to: The incumbent works closely with Case Management/Discharge Planning personnel to facilitate timely discharge of patients to maximize utilization of resources and provide more cost effective health care. Conducts concurrent reviews of inpatient and outpatient health care services monitoring the medical necessity of these services, the justification of continued stay, the effective utilization of hospital resources and the application of standards of care. Uses pre-established criteria to cyclically and concurrently screen medical records to identify quality of care issues (occurrences/incidents). Facilitates peer review by forwarding occurrences, incidents and/or death reviews to appropriate department heads. Facilitates home health care plans after initial assessment by CM/DP personnel, e.g. contact with home health care agency durable medical equipment providers and liaison with hospital nursing staff and home health care agency professionals. Facilitates acute rehabilitation placements or long-term care placements after initial assessment by CM/DP personnel, (i.e., contact nursing home, adult home, rehabilitation facilities, to expedite transfer/placement plans). Provides contacts with medical insurance providers to confirm coverage, availability of services and explore comprehensiveness of medical insurance, i.e., private insurance companies, Medicare, TRICARE, and knowledge and understanding of policy, rules and regulations governing coverage for all beneficiaries. Serves as primary liaison with Health Benefits Advisors in order to maximize utilization of resources and provide more cost effective health care. Liaisons with local Health Department and/or Social Services Department to assist in obtaining alternatives to institutionalized long-term case placement. Contacts post hospitalization patients and families to obtain data for assessing effectiveness of discharge plans. Provides feedback to the Case Manager/Discharge on cases that have problems. Assisting the CM/DP, patient, family or caregiver to obtain appropriate and timely care and/or appointment, etc. Assists in obtaining, compiling and analyzing data available from computerization of quality improvement, risk management, case management, and/or utilization management activities as requested. Assists in identifying gaps in service delivery. Participates in in-service/educational activities related to CM/UM. Will be proficient in oral communication and writing skills. Conducts special studies as indicated and in conjunction with other aspects of Performance Improvement/Risk Management and Credentialing Programs. Performs other duties as assigned by supervisor. 5.0 Knowledge required by the position: Practical knowledge of major diseases, disease processes, medical treatment of diseases and conditions, medical terminology, anatomy and physiology in order to relate to normal treatment processes and to identify care events that are variants from accepted standards. Comprehension of medical, surgical, and psychiatric modalities/treatments and the ability to identify and correlate the treatment modalities with acceptable outcomes. Knowledge and skills to be able to interact with physicians and nurses in a professional and clinically competent manner. An understanding of the interrelatedness of medical department functions and case management/discharge planning. Ability to correlate diagnostic results/procedures and treatments with diagnosis to further evaluate variances to care patterns and to assist in the continuity of care planning for patients. Knowledge of the principles, concepts, methodologies and interrelatedness of case management, discharge planning, and utilization management. Basic knowledge of statistical techniques including analysis of population demographics. Excellent decisions making skills in gathering, assembling, correlating and analyzing facts in developing constructive recommendations for resolving problems. Ability to facilitate treatment plans as developed by discharge planners. Incumbent must have knowledge of the capabilities of health facilities separate from acute care hospitals. Knowledge in areas of case management practice, social work theory, and community resources to support and assist in developing continuity of care plans for patients. Knowledge of current rules, regulations, policies and procedures related to health laws, local, state, and federal rules and regulations, JCAHO standards, and BUMED regulations related to utilization, risk, and case management. 6.0. ADMINISTRATIVE DUTIES AND RESPONSIBILITIES. Duties include, but are not limited to: Familiarization with (a) the use of computer applications, to include word processing, referential databases, spreadsheets, and graphics, and (b) communication packages and other networking programs, and (c) the use of peripheral devices such as optical scanners, modems and laser printers, Contractor personnel shall be subject to an Automated Data Processing (ADP) background check in accordance with DoD Publication 5200.2-R. The healthcare worker is required to complete the paperwork necessary for the Government to complete the background check, Contractor personnel shall comply with HIPAA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facility, Maintaining required unit records, patient charts, forms, logs, records, registers, census reports and statistical records of workload, Actively participating in the Command's Performance Improvement Plan, Attending required annual training to include family advocacy, disaster response, sexual harassment as directed, Attending Composite Healthcare System (CHCS) training provided by the Government for a minimum of four (4) hours, and up to a maximum of 40 hours, Participating in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. Should a meeting occur outside of regular working hours, the healthcare worker shall be required to read and initial the minutes of the meeting, Participating in video teleconferences (VTCs) and other meetings as required, Fully supporting military contingency requirements. 7.0. SPECIFIC MINIMUM REQUIRED PERSONNEL QUALIFICATIONS FOR CASE MANAGEMENT ASSISTANT: In addition to the requirements contained in the basic contract, each healthcare worker shall: Possess a current basic life support (BLS) certification and maintain the certification without cost to the government, and Possess one of the following, EITHER a. Have an Associates Degree in Health Information Management and at least two years experience in a healthcare environment OR b. Possess current certification from one of the following, either (a) Certified Professional Utilization Review (CPUR) from McKesson Health Solutions, or (b) Certified Professional Utilization Management (CPUM) from McKesson Health Solutions, or (c) Health Care Quality and Management Certification from American Board of Quality Assurance and Utilization Review Physicians (ABQAURP), or (d) Certified Professional in Healthcare Quality (CPHQ) from the Healthcare Quality Certification Board (HQCB), Certification maintained at the expense of the healthcare worker. OR c. Possess a minimum of 5 years of broad based health care experience inclusive of a minimum of 2 years full-time experience within the last 3 years. AND Possess working knowledge of InterQual, and Milliman & Robertson criteria, and ICD-9CM codes and basic coding principles, AND Provide three letters of recommendation written within the last two years attesting to the healthcare worker?s skills and knowledge. At least one letter must be from a workplace supervisor who has personally observed and evaluated the healthcare worker and who can attest to the healthcare worker?s competence in the ability to perform duties. The other letters may be from a clinic or hospital administrator, physician or nurse who has personal knowledge of the healthcare worker?s health care knowledge and skills in a similar environment. Reference letters shall attest to the healthcare worker?s communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include name, title, telephone number, date of reference, address and signature of the individual providing reference.
