Description
The Federal Bureau of Investigation has a requirement for maintenance of the fitness center for the Criminal Justice Information Service (CJIS) located at 1000 Custer Hollow Road, Clarksburg, West Virginia 26306. This requirement is being set aside 100 percent for small business. The CJIS Division has approximately Thirty three (33) pieces of exercise equipment and five (5) different pieces of cardio vascular exercise equipment. The period of performance will be for a one-year period from date of award, with four one-year option periods. Interested offerors are to submit a written request for Request for Quote Number RFQ-0146411 to the FBI, Construction and Laboratory Contracts Unit, Module E-3, 1000 Custer Hollow Road, Clarksburg, West Virginia 26306, Attention: Michael S. Reece, or by facsimile to (304)-625-5391 by December 9, 2004. For information regarding this solicitation, please contact Mr. Reece at telephone number (304)-625-2805. No telephone requests for the solicitation will be accepted. All equipment maintenance shall be conducted at the CJIS Facility located at the following address: Federal Bureau of Investigation, Criminal Justice Information Services Division (CJISD), 1000 Custer Hollow Road, Clarksburg, West Virginia 26306, to the attention of the individual who will be identified on the delivery order. Items shall be shipped F.O.B. Destination. (viii) FAR 52.212-1, Instructions to Offerors -- Commercial Items is incorporated by reference and applies to this acquisition. ADDENDUM FAR 52.212-1 Para. (B) Submission of offers. Submit signed and dated offers by close of business December 27, 2004, to the Federal Bureau of Investigation, Construction and Laboratory Contracts Unit, Attn: Michael S. Reece, Module B-3, 1000 Custer Hollow Road, Clarksburg, WV 26306 or facsimile at 304-625-5391. (ix) FAR 52.212-2, Evaluation -- Commercial Items is incorporated by reference and applies to this acquisition. ADDENDUM FAR 52.212-2 Para. (A) The following factors, in descending order of importance, shall be used to evaluate offers: price, and past performance. For past performance evaluation, offerors shall provide names, addresses, and telephone numbers of three customers, Government and commercial, that have utilized the offeror's services for the same or similar item during the past two years. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications--Commercial Items with offer. Offerors shall ensure that all Representations and Certifications are executed and returned as required in this synopsis/solicitation.
