Description

The Northern Arizona Veterans Affairs Health Care System intends to solicit for a contractor to provide preventative and primary medical care through a Community Based Outpatient Clinic in Holbrook Arizona. The contractor shall be responsible for providing management, facilities, furnishings, equipment, and medical care providers, along with support services such as laboratory, radiology, and short-term pharmacy support. The intended contract will be for one year with four one-year option periods. The contract will be awarded under the authority of 36 USC 8153 and pursuant to Federal Acquisition Regulation Parts 12, 13, and 14 along with Veterans Affairs Acquisition Regulation Part 873. It is estimated that approximately 700 veterans will seek services at the clinic the first year with a 10% increase each year thereafter. A solicitation will be released on, or about, November 1, 2004. Requests for copies of the solicitation should be directed to the Contracting Officer by mail, e-mail, or fax.
