SITE VISIT MEETING MINUTES F4801-04-Q-0593 INSTALL GOVERNMENT PROVIDED CARPET IN BUILDING 29 1. A site visit was conducted on 9 Sep 04 to review the scope of work for installing Government Provided Carpet in Building 29 on Holloman AFB, NM. 2. The following issues were discussed, answers to questions asked are provided. (a) The period of performance shall be 75 days after date of award. (b) Work shall be accomplished in phases. Phases shall be coordinated with the facility manager and discussed at the pre-performance meeting. Noted was that the Government shall be responsible for furniture removal and then reinstallation. (c) Work hours shall be between 7:00 AM through 4:00 PM. Different work work hours must be requested in writing to the Contracting Officer (d) Carpet installation must be installed to maximize the use of the square footage available. For example, the carpet in the hallway is run in one direction, carpet in ajoining rooms may be run in a different direction to avoid multiple seems and minimize waste. Noted was that transition strips must be installed at the breaks in pattern placement. 3. There being no further questions the meeting adjurned. 4. Offer due date remains as indicated, 15 Sep 04, 4:00 PM. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. THIS REQUIREMENT IS SET-ASIDE FOR SMALL BUSINESS CONCERNS ONLY. THE NAICS CODE FOR THIS COMBINED SYNOPSIS/SOLICITATION IS 238330, SIZE STANDARD IS $12M. RFQ NUMBER FA4801-04-Q-0593 IS ISSUED AS A REQUEST FOR QUOTE. THIS DOCUMENT INCORPORATES PROVISIONS AND CLAUSES THAT ARE IN EFFECT THROUGH FEDERAL ACQUISITION CIRCULAR FAC 2001-24, EFFECTIVE 19 JUL 2004. THE STATEMENT OF WORK BELOW DESCRIBES THE GOVERNMENT REQUIREMENT. THE GOVERNMENT INTENDS TO AWARD ONE CONTRACT LINE ITEM NUMBER (CLIN) FOR THE FOLLOWING: REMOVING ANY AND ALL OLD CARPET AND COVE MOLDING AND PROVIDE ALL LABOR, MATERIALS, TOOLS, EQUIPMENT, AND TRANSPORTATION TO INSTALL GOVERNMENT PROVIDED NEW CARPET. THE CONTRACTOR SHALL PROVIDE CUSHION, MANUFACTURED BY FOAMEX, STYLE 31-11 CX2580, DIMENSIONS ¼ INCH THICK X 54 INCHES WIDE X 60 FEET LONG, HIGH DENSITY RUBBER, 80 OUNCE, 15 SQUARE YARDS PER ROLL OR EQUAL. THE CONTRACTOR SHALL PROVIDE AND INSTALL NEW 4 INCH VINYL COVE BASE IN AREAS THAT HAVE EXISTING COVE BASE. SOME AREAS THAT HAVE WOODEN BASEBOARDS WILL REMAIN. THE COVE BASE IS MANUFACTURED BY JOHNSONITE, PRODUCT NUMBER 48, COLOR GRAY, 120 FOOT COILS OR EQUAL. THE CONTRACTOR SHALL PROVIDE AND INSTALL T CAP MOLDING AT ALL TRANSITIONS MANUFACTURED BY JOHNSONITE, PRODUCT NUMBER 48, COLOR GRAY OR EQUAL. THE CONTRACTOR MAY SUBMIT THROUGH THE CONTRACTING OFFICER A PRODUCT THAT IS EQUAL THAN THAT SPECIFIED WITH THE QUOTE. STATEMENT OF WORK: INSTALL GOVERNMENT FURNISHED CARPET AND CONTRACTOR FURNISHED CUSHION AND COVE BASE IN BUILDING 29. SCOPE: THE WORK COVERED IN THIS PROJECT CONSISTS OF REMOVING EXISTING CARPET AND COVE MOLDING AND PROVIDE ALL LABOR, MATERIALS, TOOLS, EQUIPMENT, AND TRANSPORTATION FOR GOVERNMENT PROVIDED CARPET, (MANUFACTURED BY PATCRAFT, MODEL IS CADENCE, COLOR DRUMBEAT) FROM ON-BASE LOCATION TO BUILDING 29 TO INSTALL. 1. LOCATION: BUILDING 29, FIRST AND SECOND FLOOR. 2. SUPERVISOR: THE CONTRACTOR SHALL HAVE A SUPERVISOR ON SITE AT ALL TIMES DURING REMOVAL AND INSTALLATION. 3. NORMAL WORKING HOURS: MONDAY THRU FRIDAY, 0800 TO 1600 EXCLUDING WEEKENDS AND HOLIDAYS. ANY CHANGES TO THESE HOURS SHALL BE COORDINATED WITH THE CONTRACTING OFFICE. 4. PERIOD OF PERFORMANCE: SHALL BE APPROXIMATELY FOUR WEEKS FROM THE DATE OF AWARD WITH THE EXCEPTION OF LONG LEAD ITEMS WHICH SHALL BE VERIFIED IN WRITING FROM THE SUPPLIER TO THE CONTRACTING OFFICER. 5. NOTIFICATION OF COMMENCEMENT: AFTER CONTRACT AWARD, A PREPERFORMANCE MEETING WILL BE HELD TO COORDINATE WORK SCHEDULE. 6. MEASUREMENTS: CONTRACTOR SHALL VERIFY ALL MEASUREMENTS IN THE FIELD AND REPORT SIGNIFICANT DISCREPANCIES TO THE CONTRACTING OFFICE. 7. CLEAN UP: CONTRACTOR SHALL BE RESPONSIBLE FOR DAILY CLEAN UP OF THE WORK SITE, MORE OFTEN AS NECESSARY TO MAINTAIN A CLEAN AND SAFE WORKING ENVIRONMENT. 8. APPEARANCE OF WORK SITE: THE CONTRACTOR SHALL DISPOSE OF ALL OLD CARPET AND INSTALLATION DEBRIS OFF OF HOLLOMAN AFB AND IN ACCORDANCE WITH APPLICABLE FEDARAL AND STATE LAWS. STORAGE OF MATERIALS MUST TO BE KEPT ORDERLY ON THE WORK SITE. AT THE END OF THE CONTRACT PERIOD THE AREA SHALL BE CLEANED TO INDUSTRY STANDARDS AND TO THE GOVERNMENTS SATISFACTION. 9. SAFETY: THE CONTACTOR IS SOLELY RESPONSIBLE FOR COMPLIANCE WITH THE OCCUPATIONAL SAFETY AND HEALTH ACT (OSHA) (PUBLIC LAW 91-596) AND THE RESULTING STANDARDS, OSHA STANDARD 29 CFR 1910, 1926, AND THE PROTECTION OF THEIR EMPLOYEES. IT IS THE CONTRACTORS SOLE RESPONSIBILITY TO MAKE CERTAIN THAT ALL SAFETY REQUIREMENTS ARE MET. ADDITIONALLY, THE CONTRACTOR IS RESPONSIBLE FOR THE SAFETY AND HEALTH OF ALL SUB-CONTRACTOR EMPLOYEES. THE CONTACTOR SHALL IMMEDIATELY REPORT ANY ACCIDENTS INVOLVING AIR FORCE PROPERTY/EQUIPMENT DAMAGE, OR CONTRACTOR PERSONNEL INJURIES OCCURRING ON THE JOB TO THE CONTRACTING OFFICER. ADDITIONALLY, THE CONTRACTOR IS RESPONSIBLE FOR SECURING AND IMPOUNDING EVIDENCE OF DAMAGES UNTIL THE CO HAS BEEN NOTIFIED AND CO HAS DETERMINED THE COURSE OF ACTION TO BE UNDERTAKEN. THE AIR FORCES INTEREST IS TO PROTECT ALL PERSONNEL WORKING IN OR AROUND GOVERNMENT FACILITIES. AIR FORCE SAFETY, FIRE PROTECTION AND CIVIL ENGINEER OFFICIALS MAY PERIODICALLY ENTER THE WORK SITE TO VERIFY WORKING CONDITIONS OF THE AIR FORCE PERSONNEL. IF AN IMPROPER PROCEDURE OR UNSAFE CONDITION EXISTS, WHICH PLACES AF PERSONNEL OR EQUIPMENT IN JEOPARDY THE CONTRACTING OFFICER WILL BE NOTIFIED AND THE SITUATION CORRECTED. DEPARTMENT OF LABOR, OSHA INSPECTORS ARE AUTHORIZED RIGHT OF ENTRY TO INSPECT ANY PLACE OF EMPLOYMENT OPERATED BY AN AIR FORCE CONTRACTOR. THEY ARE, FOR THE MOST PART, NO NOTICE, INSPECTIONS. IF AN AIR FORCE INSPECTOR OBSERVES A POTENTIAL OSHA VIOLATION, HE/SHE WILL REPORT THE VIOLATIONS TO THE CONTRACTING OFFICER, WHO WILL THEN NOTIFY THE CONTRACTOR. 10. PRODUCTS/MATERIALS: THE CONTRACTOR SHALL FOLLOW THE WRITTEN MANUFACTURERS INSTRUCTIONS AND PROCEDURES FOR ALL PRODUCTS AND EQUIPMENT SPECIFIED IN THIS STATEMENT OF WORK. THE CONTRACTING OFFICER RESERVES THE RIGHT TO DISAPPROVE A PRODUCT SUBMITTED AS EQUAL BASED ON QUALITY, COMPOSITION AND APPLICATION COMPARISON. 11. DESCRIPTION OF WORK: THE CONTRACTOR SHALL SCHEDULE THE CARPET REMOVAL AND INSTALLATION WITH SITE POC. NEW GOVERNMENT FURNISHED CARPET WILL BE APPLIED WITH THE DOUBLE STICK METHOD. TOTAL ESTIMATED CARPET AND CUSHION REMOVAL AND INSTALLATION IS 3100 SQUARE YARDS. TOTAL ESTIMATED COVE BASE AND TRANSITION MATERIAL REMOVAL AND INSTALLATION IS 7000 LINEAR FEET. THE GOVERNMENT PROVIDED MEASUREMENTS ARE ESTIMATES AND ARE PROVIDED FOR INFORMATIONAL PURPOSES ONLY. THE GOVERNMENT ASSUMES NO LIABILITY RELATED TO THE ESTIMATED QUANTITIES ACCURACY. THE CONTRACTOR SHALL PROVIDE SEAM SEALER ON ALL SEAMS. THE CONTRACTOR SHALL INSTALL CARPET IN ACCORDANCE WITH MANUFACTURERS INSTALLATION GUIDELINES AND OR RECOMMENDATIONS AND INDUSTRY STANDARDS. THE CONTRACTOR SHALL REMOVE AND DISPOSE OF ALL EXISTING CARPET AND COVE BASE IN A MANNER THAT RESULTS IN NO DAMAGE TO ANY PORTION OF THE WALL, FLOOR OR EXISTING WOOD COVE BASE. THE CONTRACTOR SHALL NOT BE RESPONSIBLE FOR THE REMOVAL AND REINSTALLATION OF FURNITURE. THE CONTRACTOR IS RESPONSIBLE FOR REPAIR OF ANY DAMAGE THAT DOES OCCUR. THIS INCLUDES, BUT IS NOT LIMITED TO, PATCHING AND PAINTING OF WALLS, REPAIR OF DOOR THRESHOLDS, ETC. THE CONTRACTOR SHALL PROPERLY FILL ALL CRACKS, SPALLS, AND DEPRESSIONS IN THE SUB-FLOOR WITH A MATERIAL THAT WILL BE COMPATIBLE WITH THE MANUFACTURERS RECOMMENDED PROCEDURES FOR THE TYPE OF CARPET AND ADHESIVE TO BE USED. THE CONTRACTOR SHALL PROPERLY CLEAN THE ENTIRE FLOOR AREA TO BE COVERED AFTER CARPET INSTALLATION. UPON COMPLETION OF THE INSTALLATION, THE CONTRACTOR SHALL CLEAN ALL AREAS TO ENSURE ALL DIRT, MATERIAL WASTE, AND OTHER DEBRIS ARE REMOVED AND DISPOSED OF PROPERLY. 12. OFFERORS ARE ENCOURAGED TO ATTEND A SITE VISIT ON THURSDAY, 9 SEPT 04 AT 1000 AM TO SATISFY ANY QUESTIONS AND VIEW THE WORK SITE TO PREPARE PROPOSALS. ALL INTERESTED OFFERORS SHALL CONTACT SRA MOSO OR MSGT SAMUELSON WITH THE NAMES OF ALL ATTENDING THE SITE VISIT BY 4:00 PM 8 SEPT 04 FOR DIRECTIONS TO HAFB. ALL FIRMS INTERESTED IN ATTENDING THIS SITE VISIT WILL MEET AT THE HAFB VISITOR CENTER AT THE MAIN GATE 15 MINUTES PRIOR TO THE SITE VISIT ON 9 SEP 04 AND THEN PROCEED TO THE SITE.
