PROPOSAL ARE DUE 12:00PM (NOON) ON 17 DEC 2004. STATEMENT OF WORK IN SUPPORT OF AFIOH/SDE VIROLOGY CHLAMYDIA / GC TESTING SUPPLIES AND NONPERSONAL SERVICES 15 NOV 2004 1.. DESCRIPTION OF SERVICES: The Contractor shall provide AFIOH/SDE (Epidemiological Surveillance Division), Bldg 930, Brooks City Base TX 78235-5132 with current FDA approved In Vitro Molecular Diagnostics test equipment, support service, and reagents on a per-call basis as necessary to perform Nucleic Acid Amplification Testing (NAAT) for Chlamydia trachomatis and Neisseria gonorroheae. 2.. REQUIREMENTS: The Contractor shall provide instruments via Reagent-Rental Agreement (also known as Cost per Test Agreement) capable of performing automated walk-away analyses using NAAT technology for Chlamydia trachomatis and Neisseria gonorrogheae detection in endocervical and male urethral specimens, and in male and female urine specimens. 3. ESTIMATED USAGE: The Contractor shall propose cost per test amount to include all reagents, collection devices, and drop shipment of the collection devices to stateside customers. Currently there are 60 different sites. REAGENT MONTHLY USAGE Chlamydia trachomatis 15,500 patient samples Neisseria gonorrhoeae 15,500 patient samples 4. AGREEMENT PERIOD OF PERFORMANCE: A Blanket Purchase Agreement (BPA) shall be established for a period of One Basic Year plus 4 option years. 5. Sandoval, (210) 536-5790, FAX (210) 536-5877 6 EQUIPMENT: The Contractor shall be responsible for transportation, maintenance, installation , training and servicing of equipment. The Contractor shall provide all required tools, parts and labor as necessary with repairs/replacement completed in 48 hours of service call. 7. OWNERSHIP: The Contractor shall retain title to the instruments during the entire term of the agreement. 8. MONITORING: The Contractor shall monitor equipment to ensure downtime does not exceed 48 hours for a single occurrence and there are not more than three occurrences per month. 9. LIABILITY: The Contractor will indemnify and defend the US Air Force from and against any and all third party claims, demands, actions, debts, liabilities, and attorney?s fees arising out of, claimed on account of, or in any manner predicated upon loss of, or damage to the property of, and injuries to, or death of any and all persons whatsoever, in any manner caused or contributed to by the gross negligence or willful misconduct of the Contractor, use of its product, or negligent or willful wrongdoing by its agents, servants, or employees. EVALUATION CRITERIA for AFIOH Virology Reference Laboratory Nucleic Acid Amplification Testing and Supplies The contractor will provide a written proposal in response to the RFP Statement of Work. The Government anticipates awarding a ?best value? Blanket Purchase Agreement to the Contractor whose proposal is the most technically advantageous to the Government, while price and other factors will be considered as lesser factors than technical. Technical proposals will be evaluated on the factors described below, listed in order of importance. Technical merit is more significant than price. Each contractor will be evaluated on the basis of equipment that efficiently and effectively responds to and successfully completes tasks defined in the RFP. 1. Contractor must describe in detail their instrument's capability in performing automated walk-away testing using Nucleic Acid Amplification Testing (NAAT) technology for the detection of Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (GC). 2. Contractor?s instrumentation and reagents must demonstrate exceptional productivity with a throughput of at least 552 CT/GC assays per shift. 3. Contractor's instrumentation must require no sample preparation for testing beyond proper use of required collection device. This includes no removal of container caps, no removal of specimen swabs, and no manipulation of urine samples prior to testing on the Contractor's instrument. 4. Contractor's sample collection must meet minimum stability requirements of 60 days at room temperature for genital samples collected with a swab and 30 days at room temperature for urine samples submitted in the proper collection device. 5. ESTIMATED USAGE: REAGENT ESTIMATED MONTHLY USAGE COST/TEST///Monthly Cost Chlamydia trachomatis 15,500 patient samples Neisseria gonorrhoeae 15,500 patient samples 6. Contractor must guarantee that results from automated walk-away instrument are at least 90% in agreement with those from current test in the Virology Reference Laboratory.
