- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is RFP N68470-05-R-0001. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-25. The associated North American Industry Classification System (NAICS) Code is 339113. The US Naval Hospital intends to award an indefinite quantity requirements contract for a period of one base year with four one-year options. Description of the class of products to be procured is supplies for the Synthes Plating Systems used in osteo biosynthesis. The evaluation factors used to evaluate offers are technical capability of items offered meet government requirements and lowest price. Delivery and acceptance point shall be US Naval Hospital Okinawa, Bldg 6000 Camp Lester, Okinawa, Japan T904-0103. Offeror will be required to deliver on scheduled delivery dates and call the government POC prior to expected delivery schedule for required items. This solicitation incorporates the following FAR Provisions: 52.204-6 Data Universal Numbering System (DUNS) Number; 52.212-1 Instruction to Offerors -- Commercial Items; 52.212-4 Contract Terms and Conditions -- Commercial Items; and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders --Commercial Items.
