- UNICOR intends to enter into a five (5) firm fixed price requirements type contract for locks for UNICOR Forrest City, AR, Ashland, KY, Sheridan, OR and Allenwood, Montgomery, PA. Solicitation number is FN1217-05 and this solicitation is issued as a Request for Proposal. Locks, strike plates, deadbolts, lock assemblies, lock cylinders, and key plugs. Specification information will be provided in solicitation document. Delivery Order - the minimum delivery order quantity is $500.00 per factory location. The maximum delivery order quantity is $15,000 per factory location. Variation in quantity +/- 5%. Bid Samples will be required, all offerors who have not been previously approved by UNICOR shall submit one sample of each item to Tee Cavanaugh, at 320 First Street NW, Washington, DC 20534. Samples shall be received by the offer closing date and time. Offerors who do not submit samples will not be considered for award. Offerors are advised that the Government intends to evaluate offers and award on an all or none basis. Offerors shall submit signed and dated offers to UNICOR, Federal Prison Industries, 320 First Street NW, Washington, DC 20534, and ATTN: Bid Custodian. This information shall be displayed on the outside of the envelope with the solicitation number and offer closing date. All offerors shall complete the Business Management questionnaire in Section L of solicitation document with three references. References must be for same or similar items. All Offerors must complete the ACH form and the CCR Registration, which can be found on UNICOR?s webpage at www.unicor.gov. For the ACH form click on business commercial opportunities, navigate to, procurement forms and click on ACH form and print. For CCR Registration scroll down the page to Central Contractor Registration and complete internet based form. Fax proposals are accepted at 202-353-7794, Attn: Bid Custodian, the fax cover sheet shall reference the solicitation number and offer closing date and time. All offerors must acknowledgment all amendments if any. Offers received after the exact time and date specified will not be considered. The Government intends to award a contract resulting from this solicitation to the responsible offeror whose offer represents the best value to the government. The following factors shall be used to evaluate proposals: ability to provide sample that meets specification, Past Performance and price. A written notice of award or acceptance of an offer will be mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. The solicitation will be available on Fedbizops and UNICOR?s website at www.unicor.gov or www.eps.gov on or about Wednesday December 8, 2004. No copies of the solicitation will be mailed. All responsible offerors may submit a proposal which will be considered.
