Description

This is a modification to change the closing date to 7 Dec 2004 at 12 Noon, Pacific Standard Time. This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additi onal information included in this notice. This announcement constitutes the only solicitation. Quotes are requested; however, a written solicitation will not be issued. Solicitation number W911S8-05-Q-0001 is applicable and is issued as a request for quota tions. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-25. The requirement is small business set-aside with a NAICS Code of 518111, and size standard of $21.0 million. The minimum requi rements of the commercial item are as follows: Clin 0001: Integrated Freestanding Kiosk with Printer in key-lock cabinet, with 3.5 inch receipt printer with power supply; PC/ABS Enclosure-Black; Kiosk dimensions are: Height; 47 inches, Width: 19 inches, De pth: 17 1/2 inches. Kiosk Base System which includes Intel 815e Chipset with 3D Advanced multimedia (30 frames per sec), 1 Firewire, 2 USB, 2 COM (1 Available), built-in speaker system, AC 97 Audio, built-in 10/100 NIC, 1 Parallel Port and 2 PS2; 15.1 to 1 7 inch Active Matrix LCD TFT panel with 260 NITS Brightness; Intel Pentium III 850 MHz Processor, 512 MB SDRAM DIMM, PC-100 Type; 40GB Hard Drive - Shock mounted and pretested (7200 RPM); Windows XP-Professional Edition Includes IE 6.0 and Bar Code & Mag S tripe Reader; Limited Warranty - Includes Manual and Service DC-ROM Drive; HIPPA Compliant Surface Acoustic Wave Touch Screen 60 Degree Louvered Viewing; Overhead Signage with Plexiglas Window. Perfect for ad placement. Quantity: 25 each. FOB is Destinatio n. Delivery is to be no later than 30 days after receipt of order to Madigan Army Medical Center, Fort Lewis, WA 98433-9500. Any award resulting from this request for quotations will be made using SF1449 to the successful, responsive and responsible quoter . Quotes will be evaluated on a lowest cost, technically acceptable basis. Partial delivery of complete Kiosk may be authorized. FAR Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision i s applicable, in which the term offeror (whenever it appears) shall be replaced with the term quoter. Quoters are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items and DFARS 252.212-7000, Offeror Representations and Certifications-Commercial Items, with their quotes. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. FAR clause 52.212-5, Contract T erms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.203-6, 52.219-6, 52.219-8, 52.219-14, 52.222-3, 52.222-1 9, 52.222.21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, & 52.232-33.
