- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposal s are being requested and a written solicitation will not be issued. W9124X-05-T-0001 is being issued as a Request for Quotations (RFQ). This solicitation document and incorporated provisions and clauses are those in effect throughout Federal Acquisition Circular 01-22. This combined synopsis/solicitation is set-aside 100% for small business. The NAICS code is 561320. CLIN 0001: Family Program Services The Nevada Military Department requires the following services to be performed in accordance with the information included in this notice. Acronyms/Definitions CAC-Common Access Card DEERS-Defense Enrollment Eligibility FRG-Family Readiness Group RDC-Rear Detachment Command SSM-Site Security Manager Description: Family Readiness Professionals The Contractor will facilitate training and provide administrative collaboration of the operational roles of the State Family Program Office. The contractor will provide service to work with the Nevada Guard Family Program Office and maintain the family re source centers. Provide full management responsibility over other Family Readiness Programs Professionals. Provide responsibility for the quality and activity level of the communications between state and family resource members. Provide ongoing service to the Nevada Guard Unit Family Programs and serve as subject matter experts on State family readiness and mobilization/deployments. Some local and long distance travel is mandatory. Reimbursement will be in accordance with Joint Travel Regulations. Local travel (within 50 miles of workplace) will not be reimbursed. Perform duties requiring report processing and submission of Family Resource Center, Family Resource Group Reports, CAC operations, DEERS enrollments. Manager will also act as SSM for primary sites within the location and will supervise and maintain sites outside of the immediate location. Manage and supervise volunteer staff to include Volunteer Manager, Retired Volunteer Program Personnel, mobilization volunteers and other volunteers as nece ssary. Contractor may provide on-site visits acting as subject matter expert on matters regarding the following: FRG Assistants duties and responsibilities; RDC and FRG training requirements; expectation for community resources; State FRG Assistant progr am; perform and reinforce all duties of the Family Readiness Professionals; promote and maintain contracted Family Support website reports to the State Family Readiness Director. Perform as a mission asset facilitating training, coordination, and deployme nt of plans put out by the State Family Programs Office. Additional duties may include service towards Youth Programs, Retiree Programs, Volunteer Programs and other duties as may arise. Links with the Community Volunteer Coordinator, supplement subordina te commands, battalions, and companies to recruit and maintain an active volunteer force. Coordinate formal training of volunteers and command personnel. Coordinate through local installation resources to work with the family readiness groups, and to ins truct and train units, soldiers and their families. Maintain effective communication with the FRG Volunteer Leadership Assistants, subordinate units with publishing FRG newsletters, developing telephone trees, maintaining rosters, coordinating FRG meeting s times and locations. Assist commanders with scheduling pre-deployment and reunion briefings utilizing Operation READY training materials. Work with Rear Detachment Commanders and Family Readiness Liaison Officers to ensure timely and accurate informati on is relayed to families. Act as a liaison with National Guard Family Program Office, State Family Program Office and other community agencies. Utili ze existing resources; plan and schedule technical briefings, orientations and workshops for the command, soldier and family members in the area of family readiness groups, deployment and reunion. Refer soldiers and family members, as appropriate, for mor e specialized assistance. Provide information to soldiers and family members for implementation and organization of family preparedness and readiness groups using existing resources. Utilize existing resources; develop user friendly database, approved by chain of command, to store critical information related to emergency resources, contact numbers, locations of sources, etc. for in-state use and other uses as directed by the chain of command. Ensure database is accessible at each of the sites and disse minated to the field sites. Provide referral for clients to the resource database and local community resources. Provide literature (from state and national level) to be disbursed to FRGs and units. Coordinate presentations for linking with existing com munity resources. Coordinate with State Family Program Office/community to obtain written and visual communication materials to present information to audiences with varying levels of understanding. Provide services in support of issuing, maintaining, an d collection ID Cards and other information as pertains the managing the DEERS database. Coordinate and advertise Soldier Readiness Processing and pre-deployment briefings for family members to ensure maximum participation. Participate in installation mobilization exercises where family and ID-card assistance is required. Maintain contact wi th family members of deployed soldiers using established rosters, keeping them aware of the resources available to meet their needs. Specific areas of expertise are: family readiness training, mobilization and deployment processing requirements, i.e., fin ancial preparedness, personal preparedness, (wills, powers of attorney, emergency relief, DEERS/ID Cards, benefits and entitlements), on and off post services, child care availability, medical/dental/TRICARE support to soldiers and families Maintain update d roster of military and civilian/volunteer Family Readiness Group leaders/advisors. Provide reports and other submittals to the Contract Manager on a basis as necessary to the function and/or as directed. Employee Training and Experience; the contractor shall demonstrated ability to use office automation, word processing, presentations, spreadsheets, and especially database management; to perform the duties the contractor must be able to demonstrated knowledge of Army and National Guard organization and the Family and Soldier Readiness System, demonstrate experience in military/civilian family readiness/support or related social services, be an Army Family Team Builder (AFTB) Master Trainer. It is desired that the contractor employees have experience in Army Community Service or similar military agency. (Desired) Be a notary republic. (Desired) DEERS ID Card Trained and certified, and establish and maintain a centralized management function. Typical tasks performed by this function are general task ord er support; compliance oversight; contract reports, design, and preparation, automation design and support and execution of property control systems. The Family Resource Center South requires one manager at 40 hours per week, two Family Program Professionals at 30 hours per week, and one part-time fill-in Family Program Professional at 10 hours per month. The Family Resource Center North requires one F amily Program Professional at 35 hours per week, two Family Resource Professionals at 20 hours per week, and one fill-in Family Program Professional at 10 hours per month. The government will provide the contractor with workspace, phone, fax, computer and printer, Internet access, and limited supplies needed to support the tasks identified; all to be utilized for official use only. The selected Offeror must comply with the following commercial item terms and conditions. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The selected Offeror must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The following FAR clauses in paragraph (b) of FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orde rs-Commercial Items, will apply: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, and 52.232-34.
