Description
This is a small business set-aside, combined synopsis/solicitation for commercial items service requirement. The size standard for this requirement is $21,500,000. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation may be requested by contacting Maria Buckner at 438-8583. The Scope of Work for this requirement is as follows: 1. The Contractor shall f urnish necessary management, labor, supervision, materials, equipment, transportation, and other incidental services to transport and relocate office furniture and equipment for Contracting Division of the U.S. Army Corps of Engineers, Honolulu Engineer Di strict at Fort Shafter, Hawaii. Performance period 20-21 Januray 2005. a. The Contractor shall transport types of equipment and furniture such as but not limited to the following. See paragraph 2, Schedule for specific quantities. " Automated equipment such as monitors, computers, mouse, keyboards, telefaxes, and printers. " Office furniture/appliance such as bookcases, file cabinets, tables, and boxes. " Boxes (preboxed personal effects) with dimensions of but not limited to 14 ? x 12 x 9 ? or 24 x 16 x 12 The Contractor shall be notifed three (3) calendar days prior to any changes. b. The Contractor shall provide an on-site representative to coordinate and direct all moves and serve as the point of contact with the Government. The Contractors employees shall be under the supervision and direction of the Contractor. c. The Contractor is required to submit an Accident Prevention Program Administration Plan (POD Form 248-R (Rev) 1 Mar 95 and an Activity Hazard Analysis (POD Form 184-R (Rev) 22 Apr 91) to the Government Representative for review and acceptance within two (2) days following award. The Contractor shall not perform work until the Accident Prevention Program Administration Plan and Activity Hazard Analysis have been reviewed and accepted by the Government. The U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, which describes the required safety plans and forms may be found at the following web site: http://www.usace.army.mil/inet/usace-docs/eng-manuals/em385-1-1/toc.htm. P lease note that when a medical facility or physician is not accessible within five (5) minutes of an injury to a group of two (2) or more employees for the treatment of injuries, at least two (2) employees must be first aid and cardiopulmonary resuscitatio n (CPR) certified from American Red Cross, its equivalent, or a licensed physician. The certificate shall not be older than three (3) years from date of issue, unless the currency is specified otherwise by the issuing agency. d. The Contractor shall -- (1) move items from areas where people are working in Building 200, Fort Shafter to newly renovated workspace in Building T-1507, Fort Shafter Flats. (2) move file cabinets with contents that are not packed in boxes. 2. SCHEDULE: The move shall be performed on 20-21 Janurary 2005 between the hours of 7:30 a.m. and 4:00 p.m. The floor plan of the office layout will be provided five (5) calendar days prior to commencement of the move. The Contractor will be notified t hree (3) calendar days prior to any schedule changes. Specific quanitities of office furniture, equipment, and boxes are listed but not limited to these quantities. a. Relocate items shown on the attached list from Building S-200 (first floor), Fort Shafter to Building 1507 (first floor), Fort Shafter Flats. One way distance between buildings S-200 and 1507 is approximately two (2) miles. Move will be accomplished within 2 calendar days. 3. Government personnel pack all personal effects and loose items within each individual desk and table in boxes provided by the Contractor. The Government Representative will label all relocatable items and will be in accordance to the floor plans. 4. A Government Representative will be available during the entire day of each move. 5.
