- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is solicitation n umber W91YTZ-05-T-0035. This is a 100% small business set-aside procurement. The Standard Industrial Classification Code is 4215 and the small business size standard is 1500 employees. The North American Industry Classification System Code is 492110. W omack Army Medical Center, Fort Bragg, North Carolina, 28310, has a requirement for Blood Courier Services for 9 January 2005 through 8 January 2006 with one additional one-year option period through 8 January 2007. The estimated usage is 11,000 miles per year. Please quote on the Base and Option Periods separately. STATEMENT OF WORK : The contractor shall provide transportation of whole blood units and testing samples from various mobile sites (pick-up locations) to the Ft Bragg Donor Center. The bloo d and samples will be packed by Blood Donor Center staff, according to standards, prior to pick-up by the contractor. The contractor will deliver the blood units directly to the Fort Bragg Blood Donor Center as soon as possible after pick-up. The units s hould be delivered the same day as pick-up. If that is not possible delivery will be made no later than 1000 the next morning. The contractor will be met by Blood Donor Center staff at the FT Bragg Blood Donor Center on the evening of pick-up or the foll owing morning to accept delivery of the blood units. Locations of pick-up site will vary based on the location of the blood drive. Coverage by the contractor will be limited to the pick-up locations in the states of Virginia, North Carolina, and South Ca rolina. Pick-up locations (mobile sites) currently include Fort Jackson, SC; Charleston Naval Weapons Station, SC; Charleston Air Force Base, SC; Fort Lee, VA; and Fort Eustis, VA. The contractor will maintain insurance such that, in the event the blood is not safely transported to the FT Bragg Donor Center, this insurance will cover the cost of the lost blood supply. This will not include blood lost due to improper packing by donor center staff prior to pick-up. The contractor will provide a vehicle c apable of transporting up to 12 Collins boxes full of blood and samples. In some instances, it may be required that the contractor transport supplies to the mobile site. However, this would only occur if the contractor was already contracted to travel to that site for a pick-up of blood and samples. It is now a requirement that all contractors doing business with the Department of Defense be registered with CCR (the website may be accessed on the internet at: www.ccr.gov to register). Offerors responding to this announcement shall pr ovide all information contained in the Federal Acquisition Regulation (FAR) Provision 52.212-1, Instructions to Offeror-Commercial Items. As stated in this provision, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial offer should contain the offerors best terms from a cost or price standpoint. However, the Government reserves the right to conduct discussions if later and determined by the Contracting Officer to be necessary . 52.212-2 Evaluation - Commercial Items. 52.212-3, Offeror Representations and Certifications Commercial Items; an authorized representative of the offeror must sign these representations and certifications and must be included with the offer. The resulting firm fixed price award will incorporate the requirements of the following clauses: 52.212-4, Contract Terms and Conditions Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items; with applicable FAR Clauses: 52.222-36 and 52.222-37. Evaluation factors: The Government will award a contract resulting from this solicitation to the respons ible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. GENERAL INFORMATION: Only the Contracting Officer has the authority to approve changes to this contract that would re sult in an increase or decrease in the awarded price. Monthly payment under the terms of the contract will be made in arrears.
