Smith 202-564-0096 BACKGROUND: The Environmental Protection Agency (EPA), Facilities Operations Branch, supplies silk and live plants throughout the Agency. Plants are supplied throughout the EPA Satellite Buildings, however only in the common areas. The following EPA buildings currently has either silk and/or live plants in the common areas: Ronald Reagan Building, Ariel Rios North and South, the EPA Cafeteria, and the Franklin Court Building. EPA has other satellite buildings which will be furnished with plants at a later date: East and West and the Connecting Wing, Charles Glover Building, Crystal Mall #2, Crystal Station, Crystal Gateway, Rosslyn Building, Colorado, 1310 L Street, 1110 Vermont Avenue, Woodies Building, Metropolitan Square and 1724 F Street. The project officer will add new buildings as they become a part of the EPA. As new buildings are added to the list, the contract will be modified to include the new buildings and any costs associated with the increase. Plants are also provided for the Administrator's Suite, reception and secretarial areas. If complaints are received, regarding plants that are poor looking, dead or diseased, needing moss, fertilizer or are dusty, the project officer will verify the complaint and address these concerns verbally to the contractor and follow-up in writing within five (5) working days. Following numerous complaints, documented by the project officer, the contracting officer reserves the right to terminate the contract, at the project officer's request. If the contractor is non-compliant with the contract maintenance schedule, the contract is also subject to termination. This is a two year (23 months) requirement, effective November 1, 2004 through October 31, 2005 and November 1, 2005 through September 30, 2006. Facilities management requires that the period of performance coincide with the fiscal years. The requirements contained in this contract are considered performance-based, focusing on the Agency's desired results and outcomes. The contractor shall be responsible for determining the most effective means by which these requirements will be fulfilled. In order to fulfill the requirements, the contractor shall design innovative processes and systems that can deliver the required services in a manner that will best meet the Agency's performance objectives. This performance-based requirement represents a challenge to the contractor to develop and apply innovative and efficient approaches for achieving results and meeting or exceeding the performance objectives, measures, and standards described below. The Agency will monitor the contractor's performance in accordance with the Quality Assurance Surveillance Plan as described herein. PERFORMANCE OBJECTIVES I: INSTALLATION OF PLANTS The contractor shall provide the EPA with live and silk plants, planters, potting soil, moss, fertilizer and other materials and products needed to install and maintain healthy, professional and dust free plants in the EPA's facilities. The contractor shall provide additional moss, potting soil and fertilizer as needed to continue to maintain healthy and attractive plants. All materials and products used to maintain the live and silk plants shall be environmentally safe. The plants shall be installed by the contractor in the building locations specified by the project officer. The contractor shall guarantee the life of all live plants for the period of one (1) year. Plants that expire, that are diseased, or expire due to inadequate lighting or due to the contractor's negligence during the year, shall be replaced by the contractor at their expense. All replacements shall be of the same type and size, any differences must be discussed and approved by the project officer prior to replacement. Additional live and silk plants shall be purchased for the above locations, or other new EPA locations at the project officer's request. All additional plants will be procured through the simplified acquisition or bankcard process. PERFORMANCE OBJECTIVES II:PLANT MAINTENANCE The contractor shall provide maintenance to the existing plants (see attached list) and any additional plants purchased, throughout the life of the contract. The contractor shall provide maintenance on a weekly basis. Weekly maintenance of the plants shall consist of the following: ? water plants ? dust plants (dust live and silk plants once a month) ? remove dead leaves as needed ? remove debris as needed ? treat live plants for insect or fungus infestation as needed ? fertilize plants as needed ? replace moss as needed ? stabilize plants (stand up) as needed ? add soil as needed If the contractor is unable to provide weekly maintenance, the contractor shall contact the project officer immediately, with a new date and time to provide maintenance. If the contractor is unable to obtain access to a specific area (i.e., Administrator's Office), the contractor should contact the project officer immediately with a recommendation to provide maintenance to these specific areas. The contractor shall obtain a signature from the EPA staff, verifying the date, location and what type of maintenance provided. The contractor shall notify the project officer in writing or via e-mail, within 24 hours of performing services, if there is a need for plant replacement, removal or if any plants are missing. PERFORMANCE OBJECTIVE III: MONTHLY STATUS REPORTS The contractor shall submit to the project officer a monthly status report that includes the following: ? dates and time maintenance performed ? condition of plants ? dates fertilized ? impediments encountered ? plants replaced ? plants missing The monthly report shall include the type of impediments encountered, plant replacement shall include the type and location, plant removal shall explain the reason for removal and plant missing shall include location and type. This list is not inclusive. OVERALL PERFORMANCE OBJECTIVE: The EPA's objective is to maintain healthy and attractive foilage throughout the above buildings. The Project Officer will perform random inspections of the foilage, to assure that the maintenance is being performed as stated in the requirement. The contractor shall perform maintenance on the EPA's live and silk plants. Performance Measures: 1). The contractor shall be responsible for regular maintenance, pruning, soil, mossing and fertilizing as stated in the Statement of Work. 2). The contractor shall replace plants within 48 hours. 3).The contractor shall dust the live and silk plants regularly. Performance Standards: 1). 100% of the time, the contractor shall perform regular maintenance no less than once weekly on the live plants to keep plants live and healthy. If regular maintenance is not performed as stated in the Statement of Work, the Contracting Officer will deduct $100, from the monthly maintenance cost. 2). 100% of the time, the contractor shall replace plants within 48 hours of verbal or written notification from the Project Officer or the Alternate Project Officer. If the contractor does not provide the replacement plant within 48 hours, the Contracting Officer will deduct $50 from the monthly maintenance cost. 3). 100% of the time, the contractor shall dust the live and silk plants regularly to keep them dust free with no more than two weeks of accumulated dust. If the live and silk plants are determined by the Project Officer or the Alternate Project Officer to be dusty, the Contracting Officer will deduct $20 for each plant that is non-compliant with the Statement of Work. Quality Assurance Surveillance Plan: 1). Project Officer will conduct a monthly walk through to check all plants being maintained. 2). The Project Officer shall follow-up on all requests made to replace plants to make sure they are done in a timely manner in accordance to performance standard. 3). Contractor shall secure sign off on plants, both live and silk to be maintained during the regular maintenance visit from office occupant - as designated by Project Officer.
