Small Business Set-Aside: Total Small Business
Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quotes (RFQ) is being requested and a written solicitation will not be issued. Solicitation documents incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-25. This procurement is set aside 100% for small business. The NAICS is 484220 with size standard not to exceed $21.5M. A firm-fixed price contract is anticipated. The Fort Sam Houston National Cemetery, 1540 Harry Wurzbach Rd., San Antonio, TX 78209 has annual requirements for (1) 2,768 tons of 5/8” pea gravel (692 tons will be delivered on a quarterly basis). (2) 1,600 tons of ľ” base material (400 tons will be delivered on a quarterly basis). (3) 1,260 cubic yards of topsoil (105 cubic yards will be delivered on a monthly basis). (4) 20 loads of fill sand (5 loads-30 cubic yards per truckload will be delivered on a quarterly basis). (5) 1,920 tons of #2 base (480 tons will be delivered on a quarterly basis). (6) 24 loads of ľ” limestone (6 loads will be delivered on a quarterly basis). Contract period will be for 12 months from date of award. Quotations shall be submitted F.O.B destination. All quotations are to include a complete copy of the provisions at 52.213-3, Offer Representations and Certifications-Commercial Items. The following provisions and clauses apply to this action; 52.212-4, Contract Terms and Conditions – Commercial Items, 52,212-5, Contract Terms and Conditions Required to Implement Statues and Executive Orders-Commercial Items, including clauses incorporated y reference under Section b (5)(13-19)(22)(30,) 52.212-1, Instructions to Offerors-Commercial Items, 52.219-6, Notice of Total Small Business Set-Aside and 852.270-4 Commercial Advertising, and FAR 52.204-7 Central Contactor registration. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov. Parties interested in responding to RFQ may submit their quote in accordance with standard commercial practice (i.e. quote form, letterhead, etc.) and must include the following information: RFQ number, offeror’s complete mailing and remittance addresses; discount terms; unit prices and extended total price for line items; DUNS number and Tax Identification Number, and three references for evaluation of past performance of same or similar services to Lupe Ybarra, Contract Officer by December 22, 2004, by close of Business (COB), 2:30 pm CST, or you may submit by facsimile to (210) 949-3777.
