Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number W911YN-05-T-0003 must be referenced in your response. This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2001-25, Effective 5 Oct 2004 & Class Deviation 2004-O0002 and Defense Federal Acquisition Regulation Supplement DCN 20041122 Edition. The Government intends to establish a Blanket Purchase Agreement (BPA) for this item which will be effective for the period of on or about 21 December 2004 through 30 September 2009. This synopsis/solicitation is for small business set-aside. The North American Industry Classification System code for this synopsis/solicitation is 621999 and the associated small business size standar d is $8,500,000. Item Name: Pre-Employment Physicals and doctor follow-Ups. The following are mandatory requirements for this BPA. Pre-employment physical are divided into three types based on the type job applicant will be filling. (1) A Physical consi sts of the following: (a) Hearing Test, (b) Vision Test, (c) Respiratory clearance (PFT) (d) Blood Pressure check, (e) Physical Exam by physician; (2) B Physical consists of the following: (a) Hearing Test, (b) Vision Test, (c) Blood Pressure Check, (d) Physical Exam by physician; (3) C Physical consists of the following: (a) Hearing Test, (b) Vision Test, (c) Blood Pressure check. Vendor will be required to Perform Monitoring Audiometry Exam (Hearing Test). Perform audiogram in accordance with 29 CF R 1910.95 OSHA standards and DA Pam 40-501. Perform Pulmonary function test in accordance with 29 CFR 1910.120 OSHA standards to allow the wear of respirator. Perform vision screening using TITMUS equipment. A Medical Doctor specializing in Occupational H ealth must oversee the above functions and conduct evaluations based on the above tests, medical knowledge, and practice in accordance with AR 385-10, (Same physician performing the physicals). The physician provides follow-ups and works collaboratively wi th the Florida Army National Guard (FLARNG) Occupational Health Nurse when medical issues are identified. The physician requirement will last as long as this BPA and cost associated with his/her fees will be incorporated in the overall cost. Follow-ups are defined as a re-testing of one of the above tests/exam in the event that something may have prevented the individual from passing the exam/test on that date. All services completed under the BPA shall be invoiced monthly. It is imperative that the invo ice is received by the end of the month service occurred and shall contain the following minimum information: Name of supplier, BPA number, date of service request, call number, and itemized list of services furnished, quantity, unit price, and extension o f each service, date service was provided. The government estimates, but does not guarantee, that number of pre-employment physicals will vary from year-to-year, but will average between 250  300 personnel and doctor follow-ups will be based on necessity. The Government is obligated only to the extent of authorized purchases actually made under the BPA. Individual purchases under this BPA are limited to $100,000.00 or less. A list of individuals authorized to purchase under this BPA, identified either by t itle of position or by name of individual, organizational component, and the dollar limitation per purchase for each position title or individual shall be furnished to the provider by the contracting officer.
