Description
The Centers for Disease Control and Prevention intends to purchase an AV-Elite Synergy System Conference room design, equipment and related services. System includes furniture, technology, design, installation and training, from Diversified Resource Group, Inc., 2960 Pacific Drive, Norcross, GA 30071. The AV-Elite System is a unique product that integrates furniture with technology. AV-Elite allows customers to save money by combining furniture and integration as opposed to purchasing furniture, then hiring an integration company. It comes in one turn key package. Through an extensive market research, we the CDC videoconferencing and audiovisual (AV) experts, have found this to be the only company which can provide an integrated conference room environment. This company provides a credenza of the custom size required to fit the room that houses an electronic, automated projection screen which also is appropriately sized for this conference room. Other AV equipment will also be integrated within this credenza including AM/FM amplified, and a combination DVD/VCR player, stereo speakers, wireless keyboard and muse, Extron auto switcher with audio, audio conferencing system with mic mixer, noise canceller and 4 mic/line inputs, radio-frequency touch screen remote system, and a Tandberg 880 videoconferencing unit installed at 77 degree angle, plus assorted cables. The credenza unit shall provide for laptop connectivity in addition to housing a main CPU. This unique unit shall incorporate this company's patent pending air flow and cable management system. The Government intends to evaluate offers and award a contract without discussions with Offertory providing the requested product. Therefore, the initial offer should contain the offeror?s best terms from a technical and price standpoint. This requirement is a small business set-aside. Interested vendors capable of furnishing the government with the products in this synopsis should submit two (2) copies of a quotation to the below address. The quotation shall include the itemized price, prompt payment terms, the F.O.B. Point (Destination or Origin), Taxpayer Identification Number (TNN), Dun & Bradstreet Number (DUNs), and descriptive literature of the product quoted. The quotation must reference Solicitation number 2005-Q-01742. Quotations will be due fifteen (15) calendar days from the publication date of this synopsis or January 3, 2005, by 8:30 a.m. local time, whichever is later. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to the Centers for Disease Control and Prevention, 2920 Brandywine Road, Atlanta, GA 30341, Attention: Dorothy Reed Penix. Email quotations will only be accepted if dated and signed by an authorized company representative to: dreedpenix@cdc.gov. Diversified Resource Group, Inc., is the only source known to the government that can provide a system with the above characteristics. However, if any interested party believes it can meet the above requirements, it may submit a statement of capabilities. The capability statement and any other furnished information must be in writing and must contain material in sufficient detail to allow CDC to determine if the party can fully meet the requirements herein. This action is pursuant to 41 USC 253(1) and under the authority of FAR 6.302-1. The requirement will be procured as a commercial item in accordance with FAR Part 12.102(c) under the test program. A determination by the Government not to compete this proposed requirement based on responses to this notice is solely within the discretion of the Government. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offertory - Commercials Items (MAR 2000) applies to this acquisition; 52.212-2, Evaluation - Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions - Commercial Items (MAY 1999) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24). OFFERTORY SHALL INCLUDE A COMPLETED COPY OF THE PROVISION AT 52.212-3, OFFERTORY REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS, WITH YOUR QUOTE. The best way to complete this certification is to download the FAR Clauses (Volume II) at: http://www.arnet.gov/far, print out the clause 52.212-3, Offeror Representation and Certification, and fill in the appropriate blanks.
