Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. MOBILE MEDICAL SURVEILLANCE - Contractor to provide professional non-personal medical services, lab/chemical services, equipment and supplies required to provide periodic medical surveill ance examinations and report at five locations within the state of Illinois for the Army National Guard. All services are to be performed in a mobile lab owned and operated by the contractor. See solicitation for services and tests required at each locat ion. All procedures will be conducted in accordance with the American Board of Occupational and Environmental Medicine, American Association of Occupational Health Nurses, US Department of Labor, Occupational Safety and Health Administration and the Natio nal Institute for Occupational Safety and Health Standards. A board certified occupational medicine physician and certified occupational health nurse shall be available at each site. Current estimated number of employees requiring services is 200. Medic al exam reports showing test results of each employee will be required from the contractor in electronic (disk) format compatible with Occupational Health Manger (OHM) software. The contract will be a Not to Exceed (NTE) and Firm Fixed Price (FFP). North American Industry Classification System code is 621999. The small business size standard is 500 employees. This is a total Small Business Set-Aside, however, if no small business respond with reasonable and acceptable offers, large business offers will be considered.
